Incisions from one of yesterday's minimal incision lapidus patients (I drew a Jack-o-lantern to mark the foot:)

In my opinion, this represents a significant advancement in bunion surgery: to be able to do a lapidus TMT fusion, through small percutaneous incisions, with fixation that is very strong, avoids plantar gapping, sub-cortical, gets great compression, and the patient cannot palpate the fixation through the skin....all on under 45 minutes. That is significant innovation.

There are literally millions of people walking around with a failed distal osteotomy bunion surgery. If a distal chevron was done and failed, it can still be "rescued" in most cases with a lapidus---take special care to correct frontal plane rotation.

This is a woman with a failed chevron bunionectomy done only 3 years ago, with an "instability" screw placed between the cunieforms.

I did this case MIS, with 2 stab incisions for the lapidus, and small stab incisions distally to remove the old bunionectomy screws and inter-cunieoform screw (that was clearly not controlling any hypermobility of her 1st ray).

MIS lapidus has much less post-op pain, less swelling and inflammation, can walk in a boot immediately after surgery, and can be done in under an hour.

This is a good example of a severe bunion fix. Severe angles pre op, the joint is now aligned and fixated, all the screws are under the surface of the bone and will not rub on shoes. This is the right way to fix a bunion permanently.
