Needle vs knife for a brow lift.

There are limitations to what a brow lift with Botox or any neuromodulator can achieve. A toxin-based lift works by relaxing the depressor muscles so the frontalis, which is an elevator, can dominate and lift the brows slightly. But it cannot replace true brow elevation when the issue is excess skin, brow ptosis, or structural descent.

What sets a good injector apart from a great injector is not just knowing anatomy or how to inject, but choosing the right patient for the correct procedure and setting realistic expectations. It means recognizing when a neuromodulator lift will give a subtle, beautiful result and when the patient actually needs a surgical brow lift or upper blepharoplasty to reach their goals.

Realistic expectations and honest guidance always create the best outcomes. Sometimes the needle is enough, and sometimes the knife is the more appropriate choice.

How did we get to a place where a 28 year old is getting a facelift?

Facelifts are not reversible. They are invasive, expensive, and require revisions every 10 to 15 years as the face continues to age. Yet a very small subset of surgeons (outliers) are fear mongering patients away from fillers to steer them toward the scalpel instead.

Common sense and ethical judgment feel increasingly rare in aesthetics.

As physicians, we take an oath to first do no harm. In this field, that principle can become blurred when fear is used as a marketing tool to promote irreversible procedures.

Fillers, when done correctly and safely, have a valuable place in facial rejuvenation. They help maintain structure, restore balance, support aging tissues, and allow many patients to achieve natural, refreshed results without surgery. They also help patients delay or avoid surgical interventions that come with higher risk, cost, downtime, and permanent change.

Treatment should always be individualized. A 40 year old with moderate skin laxity and volume loss may benefit more from a surgical approach, while a 55 year old with good bone structure and healthy tissue could achieve a beautiful, refreshed result with injectables alone.

It is not about fillers versus facelifts. It is about honesty, education, and using the least invasive and most appropriate option for each patient at the right time, guided by ethics and true patient care.
