Kate came to see me because she wanted a sharper jawline and a more defined neck. She did not have excess weight or fullness. Her concern was structural and genetic, with early skin laxity. ✨ When Filler Starts Changing Your Face 👀✨

Kate is already beautiful, and she came to see me feeling confused and discouraged. What started as a little chin filler slowly turned into jawline filler, cheek filler, and Sculptra — all in an effort to create definition and structure in lower face and neck. ✨ When Fillers Make You Look Older 👀✨

Many women tell me the same thing: “Fillers used to work… and now my face just looks puffy.” That experience is more common than you think - and it doesn’t mean you did anything wrong.

What’s happening is deeper than the skin. As we age, the ligaments loosen and the structural support of the face weakens. Cheeks descend, jowls appear, and the face starts to feel heavier. Filler is often expected to lift and hold tissues up - but that’s not the job it was designed to do.

Filler isn’t bad - it’s when it’s used to replace structure that has fallen overtime. Hyaluronic acid filler is soft and mobile. It adds volume, but it doesn’t recreate the firm support needed to lift the face. When it’s used to compensate for sagging, it can build up and create fullness without true lift - what many women describe as “filler fatigue.” This is why finding the right provider is very important.

🧠 Fillers are best used to restore volume in the right places, not to lift falling tissues. When structural support has weakened, the correct approach is to reposition the tissues back where they belong. In many cases, removing excess filler first allows the face to look lighter and more refreshed. Once support is restored, conservative amounts of filler or fat transfer can be used thoughtfully to refine volume - not replace surgery.

The goal isn’t to criticize fillers.

✨ Mini Facelift vs What Patients Are Really Asking For 👀✨

One of the most common requests I hear is “I want a mini facelift.”

But most of the time, patients aren’t asking for less surgery - they’re asking for a natural result that still looks like them.

In my consultations, women are usually drawn to the word “mini” for three reasons: cost concerns, fear of looking overdone, or the feeling that they’re “not aging enough” to need a full facelift yet. Those concerns are valid - but the solution isn’t a one-size-fits-all procedure.

When someone asks for a mini facelift, I listen for the underlying goal. What they’re really asking for is typically subtle lifting, hidden incisions, and correction of early jowling or heaviness without changing their identity. That’s where technique matters more than labels.

✨Endoscopic Deep Plane Facelift — Deep Plane Lift With Minimal Incisions 👀✨

An endoscopic facelift allows me to lift the deeper facial tissues without lifting any skin in front of the ear. This preserves the fat under the skin, maintains blood supply, and supports a natural, long-lasting result.

Many people ask how this differs from a traditional SMAS facelift. A SMAS approach separates the skin from the muscle and tightens the muscle farther back, but it cannot release all of the deeper ligaments that contribute to sagging. That limitation reduces the overall lift.

My endoscopic technique keeps all the work deep. I do not lift the skin in front of the ear. Instead, I release the deeper attachments, lift vertically, and secure the tissues to the deep temporalis fascia for stronger support.

With the endoscope, I can see the small facial nerves directly while working under the muscle. This gives me greater precision and helps preserve nerve function more reliably than magnification alone.

✨ What Really Happens to Old Filler During a Facelift?

Many people assume their filler is “gone” in a few months or by the time they’re ready for a facelift - but inside the operating room, I often see a very different story.

Old filler doesn’t simply disappear when the skin is lifted. It can sit superficially or deep in the tissues, and both locations can affect your results. Your body may also react a certain way while healing. Hyaluronic acid fillers attract water, which means it could lead to more swelling, longer recovery, and sometimes skin that becomes loose once the swelling finally fades.

This is why I may recommend dissolving filler ahead of time. Leftover filler can distort. Once the swelling settles, patients may notice volume loss or irregularities they didn’t see before. That may require replacing volume with fat grafting, conservative filler or additional refinements to restore balance.

So if you’re considering surgery, be prepared:
➡️ Gather your records - HA filler, Kybella, Radiesse, Sculptra
➡️ Know that fillers last far longer than most people think
➡️ Old filler can impact swelling, healing, and your outcome.
➡️ I may recommend dissolving filler.

One of the most common questions I hear is: “Will people see my scars after a facelift?”

As a facelift surgeon, I place incisions in areas where they naturally blend into the face’s existing curves and shadows. Even up close, they’re designed to be difficult to detect once healed.

Here’s how I do it:
✔️ I follow the natural landmarks around the earlobe, behind the ear crease, and into the hairline so the incision disappears into existing contours.
✔️ I customize each placement based on your anatomy, hairstyle, and skin characteristics
✔️ The goal isn’t just to hide the incision, but to create harmony so it fits your individual structure.

Every person heals differently, though. Skin type, genetics, previous surgeries, and post-op care all influence how the incision matures.
