The antimicrobial susceptibility test (AST) is unlike other clinical laboratory tests in that it is ever changing! The interpretive criteria (breakpoints) must be updated as bacteria mutate, additional antimicrobial dosages are utilized and more information becomes available (e.g., pharmacokinetic/pharmacodynamic data [PK/PD]). The role of the clinical microbiology laboratory director to manage an increasing test volume and keep pace with the ever-changing AST methodology/reporting is challenging to say the least. Here is a good summary article recently posted by ASM on breakpoints that may be helpful to some lab directors in their battle to keep pace with AST.....

Evidence about optimal treatment approaches for infections caused by antimicrobial resistant organisms changes over time. So too should clinical breakpoints for antimicrobial susceptibility testing.
