Join our team at Ivy Creek Hospice! We are seeking compassionate and dedicated Registered Nurses (RN) to serve as Case Managers at our Dadeville and Elmore locations. 

Position: RN Case Manager – Hospice
Locations: Ivy Creek Hospice – Dadeville & Elmore, AL 

What You’ll Do:
✨ Provide and coordinate patient-focused hospice care
✨ Ensure safe, high-quality nursing services that reflect our hospice philosophy
✨ Support patients and families during meaningful end-of-life care

Qualifications:
✅ Graduate of an accredited nursing program
✅ Licensed RN in the State of Alabama
✅ At least 1 year of clinical nursing experience (or completion of agency preceptor program)
✅ Strong communication, organization & teamwork skills
✅ Ability to travel between locations as needed

Volunteer Kaitlyn Bell, accompanied by her siblings, shared a beautiful musical visit with the residents at Chapman’s Nursing Home. With flute, piano, and singing, their music brought comfort, joy, and smiles. Kaitlyn has graciously done this before and takes the initiative to organize each visit herself.
