We’re grateful to once again partner with Community Table, helping ensure local families have the meals and essential items they need. To jumpstart the effort, the Bloom Flourish Thrive Foundation is donating $1,000 worth of food items—a contribution that will help us make an even greater impact together.

With recent SNAP benefit changes affecting many households, this is an important moment for us all to come together and uplift our community.

‼️Alert for Families & Caregivers

It’s been reported that Centers for Medicare & Medicaid Services (CMS) expects to spend more than $15 billion this year on expensive wound-care supplies—an area flagged by investigators for waste, fraud, and abuse. 

Older adults can be especially vulnerable when services or equipment show up that weren’t requested, or when bills arrive for care they didn’t get. If something doesn’t feel right—ask questions, check your Medicare statement, and keep an eye on unexplained charges.

Bloom Healthcare at the AAHCM Annual Meeting!

This past week, members of our Bloom Healthcare team attended the American Academy of Home Care Medicine (AAHCM) Annual Meeting—an incredible national conference that brings together home-based primary care providers from across the country. The Academy is the national voice for advancing high-quality, home-centered medical care, supporting education, research, and advocacy for providers like us who bring care directly to where patients live.

This year’s event welcomed more than 400 attendees, and Bloom Healthcare was proudly represented by:
Dr. Tom Lally, Megan Graeser, NP, Shelby Banks, NP, Laura McCrery, NP, Chelsea Goston, NP, Dr. Danielle Walz, Krista Enns, PA, Kori Eberle, and Kate Spruiell.

Our own Chelsea Goston, NP presented an academic poster—“Quality Improvement Project: Implementing A Mortality Screening Tool Post Hospital Discharge to Guide Goals of Care Conversations and Improve Hospice Admissions” - an accomplishment that showcased Bloom’s leadership in innovative, patient-centered care.

We were also thrilled to connect with many of our national partners and collaborators, including representatives from Washington who advocate for sustainability in home-based care, Keebler (AI integrations), Doctors Alliance, HCCI (provider education), Dispatch Health, BrightStar, and Dragonfly Health, among others.

Two major achievements we’re celebrating from this year’s meeting:

✨ Our Founder & CEO, Dr. Tom Lally was officially promoted to President of the American Academy of Home Care Medicine (2025–2027).

✨ Our Director of Clinical Development, Megan Graeser, NP, was named one of the Fellows in the Inaugural Class of the Academy, recognizing her leadership and contributions to advancing home-based medicine.

✨ Dr. Lally also had the honor of presenting the Robert Sowislo Person of the Year Award to Dr. George Taler, a true pioneer and champion in home-based primary care whose work helped shape this field for generations to come. Dr. Taler’s wife, Cyndy, accepted the award on his behalf—a moving moment that reflected the deep legacy of compassion and innovation that defines this community.

Our providers brought back valuable insights and best practices from their sessions—knowledge that will help us continue improving outcomes and enriching the lives of the patients we serve every day.

We’re so proud of our Bloom team for leading, learning, and making an impact on a national scale! 💜
