Bloom Healthcare is excited to officially launch in-home primary care services in Missouri next week!

For more than 20 years, our team has been caring for seniors where they live—helping them stay healthier, avoid unnecessary hospital visits, and spend more time in the comfort of home. Now, we’re honored to bring that same compassionate, high-touch care model to St. Louis and surrounding communities.

Our physicians, nurse practitioners, and physician assistants are supported by a full interdisciplinary team—including nurses, pharmacists, behavioral health specialists, and care coordinators—to ensure patients and families feel supported every step of the way.

The GUIDE Model, a CMS Innovation Program, is designed to better support individuals living with dementia and the families who care for them. Through care navigation, education, emotional support, and respite resources, this program helps caregivers feel less alone while ensuring patients receive coordinated, thoughtful care.
