Starting home care is easy. Follow these simple steps:

Step 1: Contact Us
Call our St. Michael’s office. 928-810-8011

Step 2: Intake Information
We will ask basic questions about the elder or family member who needs care.

Step 3: AHCCCS Long-Term Care (LTC)
If you already have LTC, we can help you get started.
If not, we can help guide you through the application.

Step 4: Caregiver Choice
Clients may choose a family member or we can help assign a caregiver.

Step 5: Start Care
Once approved, services can begin at home.

We Offer:
• Caring in-home support
• Navajo or English services
• Respect for elders and families
• Help every step of the way
