We are currently looking to recruit for a position of a Service Coordinator in Early Intervention. We provide full training.

The Service Coordinator is a designated primary contact between service providers and parents. The Service Coordinator helps families understand their rights in the NYS Early Intervention program and available services to address the concerns they have for their children.

Position requires candidate to have:

• Workspace at Home

• Own Printer/Scanner

• Own Computer

Responsibilities:

• Assists families to plan and obtain services appropriate to their needs.

• Coordinate and monitor delivery of services and the proper use of resources.

• Participate in the development of individualized family service plans (IFSP).

• Participates with the family in the IFSP meeting to finalize services that will be provided.

• Document and maintain written records and reports regarding service coordination activities.

• Attend and participate in continuing education, ongoing staff in-services and quarterly meetings.

• Follow mandatory guidelines according to EI Regional Policies and Procedures and Employee Handbook.

In this role, you will monitor and coordinate services as set forth by the Individualized Family Service Plan, working to ensure family satisfaction with service provision.**

Provide Ongoing Case Management services to young children (Birth to 3 years of age) with special needs and their families in community and office based settings.
Facilitate the provision of collaborative, family-centered, community-based services through the coordination and collaboration among providers of services for families with infants/toddlers with developmental disabilities.
Monitor and coordinate services as set forth by the Individualized Family Service Plan, working to ensure family satisfaction with service provision.
Work from home office and in the field. Attend home visits and meetings as scheduled at local Department of Health.
Help families of children transition between services.
Prepare and submit timely notes and other paperwork.
Provide services in accordance with New York State BEI Regulations.
Training and supervision will be provided by the agency.
Excellent communication and organizational skills. Position requires some travel within Westchester County for home-visits, meetings, and training sessions.
Learn and grow professionally with a great team of dedicated and passionate early childhood professionals.
Qualifications
Qualified candidates MUST possess at least a Bachelor’s Degree in a Human Services field (e.g., psychology, social work, education, sociology, and early childhood) .
Successful candidates will have excellent attention to detail regarding note and report submission, possess excellent verbal and written communication skills, have the ability to liaise and collaborate with professionals in various disciplines and at different levels in order to best advocate for needs of children and be willing to travel to various settings for service coordination.
Job Types: Full-time, Part-time, Commission

Salary: $1,000.00 - $4,000.00 per month
