Two baseball players, two different injuries.

1. Front sided shoulder pain at layback while pitching.
2. Shoulder compression injury sliding into second.

Player #1 after an exam has moderate Internal Rotational deficit and reduced rotator cuff strength. This leads to excess force put on the front of shoulder during the pitching motion.

Player #2 has s/sx and mechanism consistent with labrum involvement, making the overhead and layback positions painful that are needed to throw. Pain is limiting the stability and strength of the shoulder, as well as the appropriate mobility for high intent throwing.

Player #1’s rehab focus is posterior capsule and cuff mobility to increase Internal rotation, and to ramp back up to competitive pitching pain free.

Player #2’s rehab focus is to reduce pain, increase rotator cuff strength to assist the injured labrum in stability, and ramp back up to throwing pain free.

Very common injuries/issues that occur to overhead athletes.

Targeted and thorough treatment helps get our athletes back on the field competing at the highest level pain free.

Great article highlighting modifiable risk factors for the overhead athlete by Kline et. al

Metrics that increase shoulder/elbow injury:

-Flexibility of the throwing arm:
Total rotation loss >5deg
External rotation loss >5deg
Internal rotation loss 13-15deg = 4-6x increase
Flexion loss >5deg
Elbow extension loss >5deg

-Pitching Overall:
>100 pitches an outing
Higher velocity and intent
Fastball usage and high spin rate
