Admissions & Resident Care Coordinator

Make a Difference in Lives as Our Admissions & Resident Care Coordinator!
This position is well-suited for individuals with experience as a caseworker, LPN, office manager, social worker, or healthcare professional who is passionate about resident advocacy and family support.
Primary Responsibilities:
• Assist with facility tours, admissions, discharges and care planning processes
• Work with insurance companies for pre-authorization for admissions
• Work with Medicaid applications
• Act as a liaison between residents, families, and the care team
• Coordinate services with hospitals, providers, and community agencies
• Address psychosocial needs of residents and provide emotional support and conflict management to residents experiencing behavioral or emotional challenges related to dementia or other conditions.
• Maintain accurate records & documentation in Electronic Medical Records and ensure regulatory compliance.
• Be able to work with time sensitive requirements
• Collaborate with interdisciplinary team members and leadership
Qualifications:
• Prior experience in assisted living facilities, nursing homes, or home care environments is highly preferred
• Strong communication, organizational, and problem-solving skills
• Ability to work independently and manage multiple priorities
