Job Requirements
* 1 year Home Health Care experience preferred
* Current LPN Certification

Job Responsibilities
* Schedule monthly Physician appointments for residents
* Process and follow up on all Physician orders
* Review all MARs for data completion
* Conduct periodic skin assessments on residents
* Coordinate transportation for residents
* Maintain detailed notations of care provided
* Develop and oversee individualized care plans for residents
* Oversee resident care
This is a full time position.
Pay for this position is $20.00/Hr with room for expansion !
