The Medicare Chronic Care Management Program has provided patients and caregivers a resource to bridge the gap between health care systems. Dr. Downen and his team of highly skilled Case Managers have developed The Guardian Care Management Program. Providing patients with constant communication and education about their chronic conditions by serving as an intermediary between patients their medical team. The Guardian team creates comprehensive Care Plans tailored specifically to patients healthcare needs. By enrolling in the program patients will experience improved health outcomes and better cost savings.
