As of Monday, February 20th, Evergreen Sports Medicine is located at 426 Industrial Ave, Suite 130, Williston. Look for the White Cap Business Park signage, and we are positioned on the front right side of the complex.

The newly renovated, larger, modern space will allow us to treat a greater number of patients while we continue to provide the same thoughtful, individualized care to all patients in the community. Remember, you do NOT need to be a patient of Evergreen Family Health to seek care at Evergreen Sports Medicine!

We are still committed to offering appointments to new consults within 1 week, with urgent appointments for trauma/injuries available within 24 hours. Please consider us a high-value alternative to urgent care or emergency department in you have suffered an orthopedic injury.

While we appreciate referrals from Primary Care Providers, PT’s, chiropractors etc, no referral is needed.
