At this time, out-of-home care options, such as adult day programs — which are communal by nature — aren’t safe for people living with dementia. People with dementia may have trouble following social distancing guidance and are extremely vulnerable to COVID-19 due to age and other medical conditions. Adult daycare programs face significant challenges because attendees are cared for in close, group settings. We develop special programs including memory care, reorientation, special reminders, games, puzzles to keep them safe and comfortable while avoiding the feeling of social isolation and anxiety. Decreasing Emergency room visit and hospital readmission for the Elderly

We help clients discharge from the hospital to remain safe in their homes. We develop individualized fall prevention plans for these clients. Healthy habits including eating and self-care, ensure discharge instructions and medication compliance, bedsore prevention, infection control, etc. Call us for more information about these specialized programs.
