❤️ American Heart Month ❤️

For many living with chronic illness, heart health is part of an ongoing story—one shaped by changing diagnoses, long conversations, and moments that matter deeply. It’s the patient who feels their energy fade. The loved one who notices small changes. The shared hope for comfort, dignity, and meaningful time together.

During American Heart Month, we recognize that heart disease often exists alongside other chronic conditions, making care more complex—and more human.

At Blue Ridge Care, our palliative and hospice teams walk alongside patients and families through every stage of illness. Whether focusing on symptom relief, emotional support, or comfort at the end of life, our care centers on what matters most to each person.

What does community-based care look like in action?

We were honored to welcome the National Partnership for Healthcare and Hospice Innovation team for a visit to Blue Ridge Care and grateful for this reflection on the many ways our team members, volunteers, and partners support individuals and families throughout life’s journey.

On January 15, members of the NPHI team traveled to Wi******er, Virginia to spend the day with our member, Blue Ridge Care. Our team joined from Washington, D.C. and across the country to be on the ground with the teams delivering care every day — walking the inpatient unit, meeting staff, listening closely, and learning how services are designed and experienced in their community.

What we saw at Blue Ridge Care was a deep commitment to quality, compassion, and accountability. These visits matter. They keep our national policy and advocacy work rooted in the lived realities of nonprofit providers and the patients and families they serve.

🦋 Bringing Adventures Through Grief to Wi******er & Loudoun County

Adventures Through Grief is a guided support program, led by our Bereavement Counselors, that helps teens (13–17) and young adults (18–26) explore their grief through creativity, storytelling, and meaningful connection.

Through a collaborative table-top role-playing experience, participants are guided through a safe and engaging space where they can better understand their feelings, strengthen coping skills, and feel less alone after the loss of someone significant in their lives.
