67-year-old female with severe ischemia, tissue loss, and amputation of the 1st and 2nd toe, underwent multiple angiograms at hospitals and was informed of no revascularization options, with below-knee amputation recommended if foot fails to heal or continues to experience ischemic pain. Patient presented with 100% blockages in distal posterior tibial and anterior tibial arteries, with significantly compromised pedal flow. Following complete revascularization of distal tibial vessels and pedal circulation, the foot exhibited warmth and bounding pulses, resulting in another successful limb salvage outcome.
