Specific job functions include but aren’t limited to:
• Manage a caseload of clients in the assigned region. Advocate for client and family needs
• Complete and manage documentation within specified time frames such as authorization requests and plans of care
• Provide clinical and educational support to staff and clients
• Participate in process improvement plan to assure that regulatory and agency policies are followed
• Perform regular reviews of clinical staff notes and documentation
• Assist and support staff where and when needed
• Interview, hire and educate nursing staff
• Performance management for assigned home health team.
• Make decisions that add value to PHC while achieving quality and fiscal responsibility
• Establish and implement care plans for new clients
