Queens Boulevard Extended Care is looking for a Fulltime Registered Dietitian:

Accurately assess patient’s nutritional status, identify nutritional risk, develop plan of care and evaluate outcomes
Interview residents or family members, as necessary, to obtain diet history.
Develop a written dietary plan of care that identifies the dietary problems/needs of the resident and the goals to be accomplished for each dietary problem/need identified.
Participate in meal rounds to monitor intake and determine/address any issues related to food service and nutritional care.
Review and revise care plans and assessments as necessary, but at least quarterly.
Participate in comprehensive care plan meetings.
Complete MDS accurately and in a timely manner.
Periodically visit Residents’ Council meetings to determine and address any issues related to food service and nutritional care.
Review monthly weights of all assigned residents. Identify significant weight change and place appropriate interventions and documentation.
Periodically assess adequacy of nutritional care provided to residents with feeding tubes and pressure ulcers.

Job description
Queens Boulevard Extended Care Facility is looking for a Director of Rehabilitation to manage and oversee all aspects of the Therapy Department as part of an interdisciplinary rehabilitation team. Our primary focus is in delivering First-Class patient care working alongside team members from nursing, medical, social services, case management, MDS housekeeping, concierge, building services and administration. In this role, you will orchestrate and direct the therapy staff in best clinical practices by developing goals, plans, therapies and objectives. Queens Boulevard Extended Care Facility provides therapy 7 days a week, along side experienced therapists servicing 280 bed.
Responsibilities:
• Plan, develop, organize, implement, evaluate, and direct the rehabilitation services of the Facility
• Keep abreast of current federal and state regulations, as well as professional standards of practice, and make recommendations on changes in policies and procedures to the Administrator
• Review department policies and procedures, and participate in making recommended changes
• Refer resident and families to appropriate personnel, as needed, to meet resident and family needs
• Involve residents and families in planning Facility rehabilitation programs
• Assume responsibility for daily staffing and utilization with input from treating rehabilitation therapists, residents’ needs, and recommendations from the Administrator
• Maintain a productive working relationship with the medical profession and other health-related facilities and organizations
• Review and evaluate the department’s workforce and make recommendations to the Administrator
• Interpret the department’s policies and procedures to employees, residents, visitors, government agencies, etc.
• Review and develop a plan of correction for rehabilitation deficiencies noted during survey inspections and provide a written copy of such report to the Administrator
• Review departmental complaints and grievances from personnel and make written reports to the Administrator of action(s) taken. Follow Facility’s established procedures
• Participate in facility surveys (inspections) made by authorized government agencies.
• Ensure that all charted rehabilitation progress notes are informative and descriptive of the services provided and of the resident’s response to the service
• Involve the resident and family in planning objectives and goals for the resident
• Meet with administration, medical, and nursing staff, as well as other related departments in planning rehabilitation programs Requirements:
• At a minimum of a bachelor’s degree in rehabilitation. Must possess a current, unencumbered Rehabilitation Therapist License
• Must have three (3) years experience in in rehabilitative nursing practices and one (1) years’ experience as a supervisor in a hospital, nursing care facility, or other related health care facility
• Knowledge of RUGS and CMI
