Update:

All patients scheduled for well child appointments will either need to complete the self check-in or fill out a full registration form when you arrive. DO NOT pay a copay for a physical - skip that part. Remember the info you are completing is for your child and not yourself - do not change your child's name to yours! :) We are trying to eliminate as much paperwork as possible.

Updates for 1/1/2026:

Self check-in - please complete this step. It will save time in the office. If your child is scheduled for a physical DO NOT pay the copay - skip that option. When filling out the field DO NOT change the name to your name - everything is in reference to the child with the appointment. We are trying to make it so you don't have to fill out registration forms at each physical. If you wish to not complete this option, we will hand you a registration form to be completed.

Please register for the new patient portal - you will be able to get physical forms, daycare forms and school notes on the portal.
