Stepping on glass can seem minor at first, especially if the pain settles down.

But if even a tiny piece is left behind, it can stay embedded and cause ongoing discomfort or infection.

Start by cleaning the area well with soap and warm water.

If you can clearly see the glass, you can try to gently remove it with clean tweezers. But if you’re unsure, it’s better not to dig around too much. That can push it deeper.

If it still feels like something is there, soaking it in warm water with Epsom salt can help.

But here’s what really matters: Don’t wait too long to have it checked.

We prefer to see these within a few days while we can still follow the entry point. Once the skin starts to heal over, removing even a small piece becomes much more difficult.

If it still hurts, feels like something is stuck, or just doesn’t feel right, trust that feeling.

👣 Dealing with plantar fasciitis but still want to stay active?

In this short, Dr. Feldman shares general guidance on managing plantar fasciitis (PF) while maintaining activity.

🦶 In many cases, light to moderate activity may still be okay—as long as symptoms are manageable. However, if you’re experiencing:
❌ Limping
❌ Noticeable swelling
❌ Increasing or sharp pain

…it may be a sign to rest and avoid further strain.

💡 Everyone’s condition is different, so activity levels should be adjusted based on your symptoms and tolerance.

👉 When in doubt, it’s always best to get evaluated to avoid worsening the condition.

It looked like a small splinter, but it turned into something much more complicated.

She had already gone to urgent care, and they tried to remove it, but couldn’t. By the time she came in, it was buried deep in her foot and causing ongoing pain.

The problem was, it was wood, which doesn’t show up on X-rays.

So without imaging, you’re basically guessing.

We used ultrasound to track it down, almost like a GPS for the splinter. Once we knew exactly where it was, we made a small incision and carefully removed it without having to open up a much larger area.

Two weeks later, she’s healing well.

But here’s the reality
If we didn’t have ultrasound, this could have meant a bigger surgery, more tissue damage, and a longer recovery.

Not all splinters are straightforward.

If something feels stuck, painful, or just not right even after someone tried to remove it, it’s worth getting it checked properly.

👣 Orthotics are meant to help—but what if they’re causing discomfort instead?

In this short, Dr. Don Pelto explains how improper or outdated orthotics can sometimes lead to new pressure points, irritation, or pain.

🦶 As your feet change over time, your orthotics may need adjustments to continue providing the right support and alignment. Even small modifications can make a big difference in comfort.

💡 If your orthotics feel uncomfortable, don’t ignore it—an adjustment might be all you need.

👉 Regular check-ups ensure your orthotics are still working for you, not against you.

This is something we see more often than people expect.

This patient already had surgery to remove a heel bone spur, hoping it would fix her Achilles pain. And while it helped a bit at first, the pain never fully went away.

When we took a closer look with ultrasound, the real issue became clear.

Her Achilles tendon was thickened almost double what we normally see. So even though the bone spur was addressed, the tendon itself was still the source of the pain.

That’s the part that can get missed.

If you treat the bone but not the tendon, the pain can come right back or never fully resolve.

She had already tried everything before this. Physical therapy, stretching, night splints, foam rolling. Even surgery. But the pain plateaued.

So we shifted the focus.

We started a combination of focused and radial shockwave therapy to target the tendon itself and stimulate real healing. We’ll do a series of treatments, and most patients start noticing improvement around the 6-week mark.

The takeaway is simple
Pain isn’t always coming from where you think it is.

If your heel or Achilles pain hasn’t improved, even after treatment or surgery, it might be time to look deeper.

A lot of people wait too long to figure this out.

They feel foot pain, assume it’s from being on their feet too much, and try to push through it. Sometimes it gets better. Sometimes it doesn’t. And that’s where the confusion starts.

Because not all foot pain is the same.

If your pain shows up after a long day, a workout, or a busy week and then settles down with rest, it’s often overuse. Your feet are basically asking for a break.

But if the pain keeps coming back, feels stiff in the morning, or sticks around even on lighter days, that’s when we start thinking about arthritis or something more ongoing.

The tricky part is that both can feel similar at first.

The difference is how long it lasts and how it behaves over time.

If your foot pain has become part of your routine instead of something that comes and goes, it’s probably time to stop guessing and get it checked.

It might start as something minor. A little soreness after a workout, some stiffness in the morning, or discomfort after a long day on your feet.

Most people try to manage it on their own with rest, stretching, better shoes, or physical therapy. Sometimes that works. But sometimes the pain just lingers.

When symptoms stick around for months, it’s often no longer just inflammation.

The tissue itself can become damaged or degenerated, which is why it stops responding to the usual treatments.

That’s where something like TenJet may be worth considering.

TenJet is a minimally invasive, ultrasound-guided procedure that targets and removes the damaged tissue causing the pain, while preserving the healthy surrounding structures.

It’s designed for cases where plantar fasciitis or Achilles pain hasn’t improved with more conservative options.

It’s not the first step, but it can be the next step when you feel like you’ve already tried everything.

If your pain is still showing up day after day and starting to affect how you move, it might be time to take a closer look at what’s really going on.

You don’t have to settle for “this is just how it is.”

This patient is an 11-year-old gymnast who had been dealing with Sever’s disease for about four months. And if you know gymnastics, you know there’s no real “taking it easy.” Everything involves impact.

They tried just about everything first. Taping. Heel cushions. Physical therapy. Night splints. Shoe inserts. Nothing really gave him lasting relief, and it was starting to affect his ability to train and compete.

That’s when we started shockwave therapy.

We treated both heels once a week, using a combination of radial and focused shockwave. We’re now on session five out of six, and the progress has been huge.

His left heel is already pain-free.
His right heel is about 80% better.

And the biggest win? He’s back to competing.

Sever’s can be frustrating because it sits right at the growth plate, and for athletes who can’t always rely on supportive shoes, it’s even tougher to manage.

But when the right treatment clicks, it can make a big difference.

We’ll finish the final session and expect even more improvement over the next few weeks.

If your child is active and keeps complaining about heel pain, especially during sports, it’s worth getting it checked. It’s not something they should have to just “push through.”

This patient has lymphedema, which causes chronic swelling in the legs. When she first came in, her legs were much more swollen. To the point where even getting her foot into a regular shoe was a struggle.

What made the biggest difference for her wasn’t just compression socks.

It was something called a lymphedema pump.

It’s essentially a sleeve that goes over the leg and gently squeezes in sequence, moving fluid up and out of the limb. It’s not the most convenient thing.

You have to sit with it for a while, usually twice a day. But when used consistently, it can significantly reduce swelling.

And in her case, it really worked.

She shared that a few years ago, her swelling was so severe she had to wear a men’s size 12 shoe just to fit her foot. Now, she’s back in a women’s size 9. That’s a huge difference in comfort and mobility.

The key thing to know is this: It only works if you keep using it. When you stop, the swelling can come back.

We did run into a small issue where a long toenail was getting irritated during the compression, but that’s something we can manage. The bigger picture is the improvement she’s had overall.

If you’re dealing with ongoing leg swelling and feel like nothing is helping, it might be worth asking about a lymphedema pump. These are usually prescribed through a vascular or vein specialist and can sometimes be covered by insurance.

Sometimes the right tool can completely change how you move through your day.

This might sound surprising at first, but for some patients, this is actually the simplest solution.

This patient had a severe hammertoe that was constantly rubbing against his shoe.

He had already tried padding, different shoes, and other treatments, but nothing gave lasting relief.

The toe was sitting on top of a bunion, creating ongoing pressure and pain every time he walked.

At that point, we had two options.

One was a more complex surgery to correct the bunion and straighten the toe. The other was a much simpler approach, removing the second toe to eliminate the source of pressure altogether.

He chose the simpler route.

And the result? Less pain, easier walking, and surprisingly, most people don’t even notice the toe is missing.

It’s not the first option we jump to, but in the right situation, it can be a very effective one.

If you have a hammertoe that keeps rubbing, hurting, or limiting what shoes you can wear, there are options. The key is finding the one that fits your goals and your lifestyle.
