Wurtland Nursing and Rehabilitation We provide a warm, welcoming environment and highly individualized skilled nursing care. Our Care Team is specially-trained to work with seniors.

Located in the small town of Wurtland, Kentucky, Wurtland Nursing & Rehabilitation offers compassionate 24-hour Skilled Nursing care in our 126-bed facility. We listen, we understand, and develop personal therapy and care programs designed to maximize your abilities and life. This includes assisting with daily living activities and skills, such as balance training, medication management, IV therapy, and diabetes management. Should you need support services, know that we can arrange everything from mental health consultations to x-rays, lab work to podiatry services, as well as private home and hospice care.

🌟 Congratulations to Trent, our 2025 Care Team Member of the Year, and to Jade our Care Team Member of January 2026 at Wurtland Nursing & Rehabilitation! 🌟

Trent joined our team in May 2025, bringing strong knowledge, dedication, and a tremendous work ethic. By August, he stepped into the role of Maintenance Director, and he has been a true blessing to our community ever since.
He ensures everything in our facility runs smoothly—keeping the lights on, water flowing, and heat and air systems functioning with care and precision. Trent always puts our residents first and is quick to lend a helping hand to staff whenever needed. His positive attitude and steady presence make a meaningful impact every single day.
We are incredibly proud to call Trent one of our own. Thank you, Trent, for everything you do for our residents, team, and community! 💙❤️

Jade has been a Physical Therapist for seven years and joined the Wurtland community in July 2025 through Reliant Rehabilitation Services. In December 2025, we were thrilled to welcome her officially to our Wurtland team.
Jade works tirelessly to support each resident’s therapy journey, always striving for the very best outcomes. Her dedication, encouragement, and genuine belief in her residents’ success shine through in everything she does.
We are so grateful to have Jade as part of our team—her hard work and positive spirit make her truly deserving of this recognition. Congratulations, Jade! 💙❤️
