Position Summary:
We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our home health team. The LPN will provide direct patient care in the home setting under the supervision of a Registered Nurse (RN), following the plan of care and ensuring high standards of clinical excellence.

Key Responsibilities:

Provide nursing care to patients in their homes.
Assist with implementing and following individualized care plans under the supervision of an RN.
Educate patients and caregivers on disease processes, medications, and self-care techniques.
Document patient care accurately and timely in compliance with company policies and regulatory requirements.
Collaborate with interdisciplinary team members to ensure continuity of care.
Respond to patient needs and emergencies as appropriate.

Qualifications:

Current and unrestricted LPN license in the state of Oklahoma.
Minimum of 1 year of nursing experience (home health experience preferred but not required).
Current CPR/BLS certification.
Reliable transportation and a valid driver’s license.
Strong clinical, communication, and organizational skills.
