📚✨ Building Bridges Through Education & Collaboration ✨📚

This week, the team from Edenbrook Yeadon Skilled Nursing Facility had the pleasure of hosting a Lunch & Learn at the main campus of the Hospital of the University of Pennsylvania!

We were honored to spend time with such an incredible group of dedicated social workers — sharing resources, discussing seamless transitions of care, and strengthening the partnerships that make a difference for patients and families every day. 🤝
