Margaret is a wonderful example of how our program helps bridge a critical gap in care.

After a recent hospital stay, Margaret did not meet inpatient criteria, which meant her insurance coverage wasn’t an option — but she still needed therapy, skilled care, and support to safely regain her strength and return home. Going straight home wasn’t safe, and the emergency room wasn’t the right place either.

That’s where Urgent SNF made the difference. Urgent SNF offers direct admission for individuals who may have had an ER visit, been under hospital observation, or are at home needing short-term skilled nursing, rehabilitation, or medical stabilization — even when traditional insurance coverage doesn’t apply.

Because of this program, Margaret received the care she needed, regained her independence, and was able to return home safely!

This is what innovative, patient-centered care looks like — meeting people where they are and helping them get back on their feet when they need it most.

Because great care goes beyond treatment, this is .

💕 February is American Heart Month – A Time to Prioritize Heart Health 💕

Heart disease remains the leading cause of death in the United States, affecting millions of lives across all ages, genders, and ethnicities. This February, take action to protect your cardiovascular health and the health of those you care about.

Here’s how you can get involved:
• Learn CPR and become a lifesaver. More than 350,000 out-of-hospital cardiac arrests occur annually in the U.S., and nine out of every 10 people die without immediate CPR. By learning Hands-Only CPR, you could be the difference between life and death.
• Go red. Wear red on National Wear Red Day (February 6th) to raise awareness for cardiovascular disease and support women’s heart health and support American Heart Association
• Adopt heart-healthy habits. Up to 80% of heart disease is considered preventable. Use this month to focus on eating a heart-healthy diet, increasing physical activity, managing cholesterol and blood pressure, and maintaining a healthy weight.
• Participate in community events. From heart walks to educational talks, look for local events recognizing American Heart Month and invite friends, family, and colleagues to join you.

Take the first step today. Whether it’s learning CPR, scheduling a health checkup, or committing to a new wellness habit, every action counts.

Your heart health matters – and so do you.

We stand for unity, equality, and love—because together, we can overcome any darkness and light up the world. Share a photo of a starry night or a moment when you saw the light in the darkness.

Let's celebrate MLK Day by shining our lights together.
