Compassion has no limit, kindness has no substitute!

Our hospice care provides compassionate care designed to surround families with love and support at one of life’s most challenging times. Acacia Hospice Care includes various services to address the unique physical, emotional, and spiritual care each person needs.🤗🌹

Who is Eligible?
People that have been diagnosed with a life-limiting condition with a prognosis of six months or less to live with their disease running its normal course. People that have frequently visited the hospital in the last six months, progressive weight loss, recurrent infections, skin breakdown and specific decline in condition. There is no out of pocket expense for hospice appropriate services.🪷

In many cases, patients have not been given a six month or less prognosis by their physician, but begin to experience changes in their condition and show signs of decline. An in-person hospice evaluation to review recent medical history and change of condition can help determine if a patient is, or will soon be, eligible to receive hospice care.

How to Get Started?

If you have received a hospice referral from a doctor, then it’s time to talk to a hospice provider about your options.

If you haven’t been certified through a medical provider, you can still reach out to speak with our team.

Acacia Hospice Care has dedicated clinical staff on call 24 hours a day to answer questions and provide assistance. We provide services, based on the patient and their medical needs, seven days a week, 365 days a year.
