5 Things to Bring to your DOT Physical

1. A valid driver’s license and alternate form of picture ID
2. Eye glasses, contact lenses, and hearing aids if you use them
3. A complete list of medications you’re currently taking and contact information for the prescribing doctors
4. If you have medical conditions that could possibly cause issues, you’ll want to ask your primary care physician for a medical release letter stating that they won’t prevent you from operating a commercial vehicle safely
5. An FMCSA waiver if you’ve been exempted from test requirements before
