Rachelle is a Sherburne and Wright County Caregiver. She joined CHC in May of 2023.

Rachelle has been selected as the employee of the month as she stands out for the heart she has for her clients. Rachelle goes above and beyond by helping out on nights and weekends when her clients are in a pinch, she is always there for them when they need her and is always willing to adjust her schedule to meet her clients needs.

We also very much appreciate Rachelle's ability to always communicate with the office. We feel blessed to have Rachelle on the CHC team! Keep up the great work Rachelle!

Meet Gwen, one of our fantastic full-time Anoka county caregivers! She officially joined the CHC family in February of 2025, and we are so glad she did!

From the moment she walked through our doors, Gwen has been an absolute superstar. She's a communication wizard, keeping our office in the loop, and she's super adaptable and ready to jump in and connect with all sorts of clients. Gwen is very reliable, always making sure to meet each client's individual needs with a smile. Need a sub? Gwen's our go-to hero, always willing to lend a hand and assist clients in any way she can. We're incredibly blessed and so grateful to have her sparkling personality on the CHC team!

We want to recognize Kari for her exceptional dedication to her clients. Kari joined CHC in October 2024 and provides care for clients in Wright County. Kari consistently goes above and beyond to support her clients. Recently, she was concerned about a client's well-being after a difficult morning shift. She took it upon herself to check on the client after hours, and upon arrival, she realized the client needed immediate medical attention and called the paramedics. Her proactive care and concern saved this clients life.

In addition to her outstanding client care, Kari is always willing to pick up extra shifts when her schedule allows and maintains open communication with the office regarding any questions or concerns. We thank you, Kari, for your hard work and for everything you do for your clients each and every day. We, and your clients, truly appreciate you!

We are offering a selection of gently used medical supplies and incontinence products to individuals in need. Prior to donation, we invite inquiries regarding the availability of these items. Collection is available Monday through Friday, between the hours of 8:00 a.m. and 5:00 p.m., at our Zimmerman office.

Our current inventory includes: eleven twenty-count packages of Tranquility Incontinence Underwear (Heavy Flow, size medium); three twenty-count packages of FitRight Incontinence Underwear (size medium); four twenty-count packages of FitRight OptFit Incontinence Briefs (size medium); four fifteen-count packages of Depends Night Defense Underwear (size medium); two packages of bathing cloths; three twenty-eight-count packages of Contour Incontinence Bladder Pads (Super); one thirty-six-count package of Poise Overnight bed pads; and three ten-count packages of Chuk's bed pads. Additionally, we have available a pre-owned bed rail, a gently used shower chair, a new shower bench, a gently used walker, and a toilet seat riser.

Your excellent communication and chart documentation assist us to help you meet your clients needs. We appreciate your caregivers heart and your desire to go above and beyond for your clients. We are amazed by the way you are able to balance your schooling with your client load while at the same time keeping your clients a priority. You are a gem and we are so happy to have you on the team.
