We do this by providing five essential health programs: Prescription Assistance, ED Care Coordination, ACA Marketplace Health Insurance, EarnBenefits and an Indigent Fund.

The Prescription Assistance Program ensures those without prescription health insurance can access medications needed to maintain a healthy lifestyle and reduce unnecessary / non-emergent ED use. In 2016, Spring Creek Health assisted 850 patients access 15,279 free /reduced prescriptions valued at $5.2 million. To date, we have assisted more than 5,700 patients connected to 243,835 prescriptions valued at $44.2 Million.

ACA Health Insurance Certified Navigator enrolled uninsured patients into Marketplace health insurance from 2013-17. Spring Creek Health enrolled 92 consumers in OE1, 325 in OE2, 123 in OE3, and 145 in OE4 for a total enrollment of 687.

EarnBenefits (EB) began March 2014 to screen consumers for 20+ federal, state, and local benefits to ensure social and health service needs are met. From 2014 - 2016, EB Counselors screened 1,731 consumers for 43,537 programs and enrolled 1,315 consumers into 4,041 services valued at $5.2 million of which $1.4 million was health insurance.

The Emergency Department (ED) Care Coordination Program provides Nurse Care Coordinators to manage self-pay patients who are utilizing the ED as a medical home. Since 2014, ED Care Coordinators have managed 5 ED in South Georgia saving hospitals more than $1.4 million.

SCHC Indigent Fund was created to assist ED patients who were uninsured and indigent access a primary care doctor and free and/reduced cost prescription medications. From July 2016 to July 2017, we have assisted 41 total patients: 39 patients accessed 590 prescriptions and 13 patients saw a primary care doctor. The indigent fund has provided $4,245 for these services. The value to the patients exceeds $547,700!
