Cardiovascular disease remains the leading cause of death globally, responsible for nearly 20 million deaths every year. Yet it rarely occurs alone. Many people also live with conditions such as:

đź’” Hypertension

đź’” Diabetes

đź’” Obesity

đź’” Mental health disorders

đź’” Chronic kidney disease

Today, the World Heart Federation launched a new Roadmap on cardiovascular disease and multiple long-term conditions, calling for health systems to treat the whole person, not just one disease at a time. Why does this matter?

🌍 Around 37% of adults globally live with multiple health conditions.

đź’Š Many patients take several medications and see multiple specialists.

🔄 Care is often fragmented and difficult to navigate.

Integrated care brings healthcare professionals together so patients can receive coordinated, person-centred care that reduces hospital visits and improves quality of life. Learn more at the link in our bio.

A special thank you to our lead partners, and AstraZeneca for making the Roadmap possible, and to Novo Nordisk and Roche for supporting the Roadmap Summary.

Know Stroke Prevent Stroke

The Montana Cardiovascular Health Program conducts statewide activities to prevent stroke and associated risk factors.

Our goal is to reduce the public burden caused by stroke and the impact of death and disability on Montana communities and families.

The program sponsors media and outreach education campaigns to increase awareness of the signs of stroke and the need to call 9-1-1 fast, and to encourage individuals to prevent stroke through healthy behaviors.
