Case history of Endometriosis:

A 32-year-old nulliparous white female presents with a history of progressively worsening menstrual pain that is now causing her distress for most of the month. She misses 2 to 3 days of work each month. She finds no relief from ibuprofen and can no longer tolerate the headaches associated with her contraceptive pills. She is currently sexually active with her long-term partner. Her relationship is being affected by associated stress and pain during in*******se. On vaginal examination, her pelvic musculature is moderately tender. Her uterus is of normal size and minimally tender. Rectovaginal examination reveals uterosacral nodularity and exquisite tenderness. Stool is soft, brown, and haeme-negative.

🎯Discontinuation of medication before surgery:

° Aspirin- 7 days before surgery

° Clopidogrel- 10 days before surgery

° Warfarin- 5 days before surgery

° Newer oral anticoagulants( eg. Rivaroxaban,Dabigatran) : 2-3 days before surgery

° Unfractionated heparin- at least 2 hours before surgery

° Beta blocker & lipid lowering drug: should be continued

° ACE inhitor & ARB blocker-often omitted 24 hours before surgery

° Anti convulsant drug: should be continued

° Anti parkinsonism drug: should be continued

° Estrogen containing oral contraceptive : 4 weeks before routine operation

°Progesterone only pill: should be continued.

[** In high risk of thrombosis Patient -Aspirin can be continued.

Warfarin therapy should be justified according to risk of bleeding & risk of thrombosis

smoking should be stopped at least 8 weeks before surgery for maximal benefit.]
