Whilst the number of people with diabetes has grown exponentially, and it is affecting younger people and even children where it used to only be a problem amongst older people. But what about insulin resistance? Where does it fit into the picture?

Insulin resistance, diabetes, and obesity are some of the biggest as well as the fastest spreading health disorders in the world – and they all go hand-in-hand. But when we look at them, they all have one thing in common: Insulin.

But what is insulin? What does it do? And why can it cause diabetes and obesity?


What is the function of insulin in the body?
First things first: Insulin is a good thing. It is the hero in the fairytale of our health. It is when we do things that make insulin ineffective or non-existent that it becomes the middle point of many health problems.

Insulin is a hormone produced by the pancreas, and its main function is to regulate the sugar in our bloodstream. A waist size of more than 102cm for men and 89cm for women.
High levels of triglycerides (a type of fat in the blood).
Lower levels of magnesium.
Sodium is retained, which leads to holding excessive water in the system, which causes high blood pressure levels.
High amounts of inflammatory compounds in the bloodstream. This can lead to damage to the blood vessel walls and help cause blood clots which can cause heart attacks and respiratory failures.
Low HDL level (Less than 40 mg/dL for men and 50 mg/dL for women).

Low blood sugar can be defined as follows:

What causes low blood sugar levels (hypoglycemia)?
Wondering what causes low blood sugar levels? When functioning normally, the pancreas excretes the correct amount of insulin needed to keep the blood sugar levels balanced. However, when a person suffers from low blood sugar, the pancreas releases too much insulin which causes the blood sugar levels to become too low. This, along with other diseases, can cause hypoglycemic episodes.
What should my blood sugar levels be?

What causes hypoglycemic episodes in non-diabetic people?
Reactive hypoglycemia:
This is the most common reason for low blood sugar levels in people who do not suffer from diabetes. This is when the pancreas releases too much insulin due to a sudden increase in glucose in the bloodstream, such as after a big meal containing lots of carbohydrates. The insulin is still present in the blood after several hours of the meal, which is not as it should normally be.

Reactive hypoglycemia often affects people who are overweight and people with type-2 diabetes. There is evidence to propose that reactive hypoglycemia can lead to type-2 diabetes.

Symptoms of Low Blood Sugar(Hypoglycemia)
What is happening during a hypoglycemic episode?
The following can be symptoms caused by hypoglycemia:
Paleness in the face similar to when one is feeling nauseous
Shaking as can be clearly seen in the hands
Perspiration without much physical effort
Feeling weak and deprived of energy
Increased heartbeat
Constant hunger
Being agitated for no apparent reason
Having trouble concentrating

Care and Prevention of Low Blood Sugar (Hypoglycemia)

In case all three of the criteria listed below are met, you should be referred for further evaluation at a healthcare facility:
Blood glucose level lower than 2.5mmol/l (or sometimes slightly higher.)
A few or all of the symptoms are listed above.
The symptoms disappear within 10 minutes of consuming sugar.
How is the diagnosis made?
Hypoglycemia can be diagnosed simply by measuring the blood glucose level. In the case of it being lower than, 2.5mmol/l, hypoglycemia is diagnosed.
If the doctors suspect a tumour might be present in the pancreas, the patient will be taken up into the hospital for close observation. Here, the glucose levels are closely monitored, usually for 3 days.
Should these levels drop below a certain point, the medical staff measures the insulin level in the bloodstream.
If the insulin levels are found to be high, it indicates a tumour in the pancreas, which will be looked for, and if found, removed.
Exercise and diet
Exercise lowers the blood sugar levels but will not normally cause hypoglycemia in individuals who are overall healthy. If you begin to experience symptoms of hypoglycemia while doing physical exercise, you should eat some complex carbohydrates such as starch or pasta before beginning. Blood sugar levels can be kept up by consuming small, steady amounts of simple carbohydrates such as energy drinks or energy bars during exercise. If you have been diagnosed with reactive hypoglycemia, the first thing to do is change or adapt your diet to help manage the hypoglycemia. It should include a lot of complex carbohydrates such as potatoes, rice, pasta, etc. The dietary routine should also be changed. It should consist of more, but smaller meals: breakfast, lunch, dinner and three in-between meals. This helps avoid big fluctuations in insulin secretion from the pancreas.
Treatment of hypoglycemia:
Reactive hypoglycemia is treated with dietary changes as stipulated above.
Blood sugar levels should be checked regularly. If blood sugar levels are lower than normal, but there are no clear symptoms of hypoglycemia, just continue with normal routines to keep blood sugar levels balanced.

Most of the time, the symptoms of gout are very sudden and painful. It often occurs in the middle of the night and can be so painful that it wakes you from your sleep. Symptoms include:

Extreme joint pain
The most common place where gout is found is in the joint at the base of the big toe. It can however occur in many other places such as the rest of the feet, ankles, knees, hands and fingers, and of course the joints located in the wrists.

It has been found that the pain is at its worst round about 12 to 24 hours after it has first started.

Constant discomfort
After the worst pain has gone away, discomfort is still felt in the joints. This discomfort might be present for a few days or sometimes even a few weeks. Gout that appears after the initial attack is likely to last longer and be more widely spread than the first time.

Redness and inflammation
The involved joints become swollen sensitive and red.


When should a doctor be consulted?
As soon as you feel a very sudden and extreme pain attack in a joint, it is best to contact your doctor. If gout is left untreated it can lead to even more pain and permanent damage to the joint.

Gout is caused by the accumulation of urate crystals in a joint, which results in inflammation and extreme pain.

Urate crystals form due to high levels of a substance called “uric acid” in the bloodstream.

Uric acid is produced by the body when it breaks down purines, which are substances that can be found in the body. Purines can also be found in certain types of food namely: organ meats, herring, anchovies mushrooms as well as asparagus. Other foods that lead to high levels of uric acids are those that get turned into high concentrations of glucose in the body. These include sugary drinks, bread, white rice and pasta. This also includes beverages that contain alcohol, as alcohol also results in high amounts of glucose.

Under normal circumstances, uric acid will dissolve in your blood and will get filtered by the kidneys whereafter it is excreted from the body through urination. Recurrent gout
Some people may never experience any symptoms of gout after the first time, whereas others may suffer from the effects of it several times per year. People with recurrent gout can prevent gout attacks from returning by using medication.

Advanced gout
Gout that is left untreated can cause urate crystal deposits to form underneath the skin in nodules referred to as “tophi” (pronounced TOE-fi). Tophi can develop in a variety of parts such as your fingers, hands, feet, elbows or Achilles tendons. Tophi usually are not painful, but during gout attacks, they become sensitive and swollen.

Kidney stones
People with gout may experience urate crystals forming in the urinary tract, resulting in kidney stones. The risk of developing kidney stones can be reduced by using the correct medication.

Conventional Gout Treatment
A complete cure for gout has not yet been found, but gout can be greatly reduced through several conventional methods. Most of the time doctors prescribe non-steroidal anti-inflammatory drugs (NSAIDs) such as “ibuprofen” which helps lessen the inflammation and pain.

Corticosteroids are used to gain a similar effect; these are given by means of oral medication (pills) or injections. Other medications help lower the levels of uric acid in the blood with the best known probably being allopurinol (Zyloprim). Note that these methods should be used as a last resort since they can all cause significant side effects.

The following factors should be taken into account:
Meats that contain high levels of uric acid should be avoided. These meats include anchovies, sardines and organ meats. Lifestyle factors
Day to day lifestyle choices could increase the risk of developing gout. One of the main factors that are often overlooked is alcohol consumption. Excessive alcohol use is generally defined as more than 2 drinks per day for men and more than 1 drink per day for women.

Medical conditions

In men, however, gout tends to develop between the ages of 40 and 50 years instead of later in life.


Tests and diagnosis of gout
Different tests exist which can be used to diagnose gout, such as:

Joint fluid test
A needle can be used to extract fluid from the affected joint. This joint fluid is then examined under a microscope where, if present, urate crystals can be identified.

Blood test
Blood tests are done to measure the uric acid levels in the bloodstream. This cannot solely be trusted to determine gout, as some people have high levels of uric acid, but never develop gout. The opposite is true, where some people show symptoms of gout but their uric acid levels are not abnormally high.
