Up to 90% of women with endometriosis report gastrointestinal symptoms such as bloating, cramping, diarrhoea, constipation, or nausea, particularly around menstruation.

In many cases, these symptoms are attributed to IBS and treated accordingly, delaying an endometriosis diagnosis by years.

The reason is complex. The gut and reproductive system share nerve pathways, and endometriosis lesions can sometimes implant on the bowel. When this happens, bowel movements can trigger severe pelvic pain, not because the gut is the problem, but because the bowel is affected by endometriosis-related inflammation.

Researchers are also studying the gut microbiome and its possible link to endometriosis. Early evidence suggests that women with endometriosis may have a different microbiome profile, which could influence oestrogen metabolism and inflammation.

Emerging research suggests the immune system may play an important role in endometriosis.

In a healthy body, endometrial cells that move outside the uterus are usually identified and cleared. In women with endometriosis, the immune system may not remove these cells effectively, allowing lesions to grow.

Studies have also found higher levels of inflammatory markers and changes in natural killer cell activity in women with endometriosis. These immune cells normally help clear abnormal cells.

This may help explain why endometriosis is sometimes linked with other autoimmune and inflammatory conditions.
