Part of Discovery Health’s DCC network (Diabetes Care Coordination). A dedicated multidisciplinary diabetic team consisting of GP Diabetologists, a lifestyle coach, a diabetes educator, a podiatrist, a biokineticist, dieticians, ophthalmologists, etc.
