Myofascial Pain Syndrome / Muscle Trigger Points "knots".

Myofascial pain syndrome is a common condition affecting the muscles and associated soft tissues. Pain radiates from one or more trigger points stimulated by pressure, stress, repetitive motions, work or overuse.

Sensitive areas of tight muscle fibers can form in your muscles after injuries or overuse. These sensitive areas are called trigger points. A trigger point in a muscle can cause strain and pain throughout the muscle. When this pain persists and worsens, it is characterized as Myofascial Pain Syndrome.

Common areas where these trigger points are found are the shoulders, neck and shoulder blade region. Pain can be dully achy to burning in sensation. Pain can fluctuate in intensity and duration.

Chiropractors diagnose and treat trigger points and Myofascial Pain Syndrome.

Sciatica is described as nerve pain from an injury, inflammation or irritation to the sciatic nerve, which originates in the buttock/gluteal area.

Sciatica is not a condition, but rather a symptom of another problem involving the sciatic nerve. True injury to the sciatic nerve “sciatica” is actually rare, but the term “sciatica” is commonly used to describe any pain that originates in the lower back and radiates down the leg.

People who have “sciatica," experience mild to severe pain anywhere along the path of the sciatic nerve – that is, anywhere from the lower back, through the hips, buttocks and/or down your legs. It can also cause muscle weakness in the leg and foot, numbness in the leg, and/ or pins and needles. The pain may be sharp and shooting or burning and cramping in nature. The pain may be constant or intermittent and is usually more severe in the leg compared to the lower back.

Dry Needling (DN)/ intramuscular stimulation is a therapy whereby a trained practitioner places thin filiform needles into the skin, targeting trigger points in the muscle (please see our previous post "trapezius myalgia, where we briefly discuss trigger points).

Trigger points, otherwise knows as muscle "knots", are raised inflamed nodules along taut spasmic bands of muscles. Trigger points generally cause pain in a muscle or referred pain which may be felt in other areas of the body, this is generally due to constriction of small nerves and/or blood vessels traversing the area.

Trigger points can develop in any skeletal muscle throughout the body, common areas are: the shoulder, arm, forearm, lower back and buttock, thigh and the calf muscles.
The needle/s are generally left in the muscle for around 5 to 7 min (condition dependent), occasionally the practitioner may move or twist the needles in order to stimulate the trigger point. One may feel the muscle twitch and a mildly uncomfortable sensation (depending on ones pain threshold).

Although similar needles are used, dry needling is NOT to be confused with acupuncture. Each practice is rooted in different literature and science. Dry needling is focused on neuromuscular dysfunction with intent to relieve pain and stiffness caused by muscular trigger points, whereas acupuncture treatment is based on normalising the flow of Qi and focuses on energy meridians / pathways throughout the body.

Who may benefit from dry needling?

Anyone suffering from tight muscles, muscular pain, trigger points and headaches caused by trigger points. Athletes with muscular imbalances and/ or pain as a result of trigger points. Tendinitis, rotator cuff injuries, strains / sprains, sciatica, plantar fasciitis, knee pain and more.

What will I feel after my session?

Depending on the patients pain threshold one MAY experience mild to moderate discomfort or post treatment tenderness for 1 to 3 days after dry needling. This is a normal response to the procedure. One is still able to go to work and continue normal daily activities. Once the tenderness has resolved the muscle will feel relaxed and pain in the area should diminish / resolve.

A registered Chiropractor will be able to assess one's condition, determine if dry needling is a suitable treatment approach and if one is a candidate for the therapy.

Trapezius myalgia (TM) is a condition characterized by pain, stiffness, and tightness of the upper trapezius muscle, generally as a result of trigger points (TP) located in the trapezius muscle.

It is further characterised by acute or chronic neck-shoulder pain.

Trigger points are raised inflamed nodules along a band of muscle. They’re one of the most common long-term muscle disorders and can affect anyone. Trigger points can develop in any muscle but commonly occur in the trapezius muscle.

One may be able to feel the raised trigger point in the affected muscle. They may feel like a knot in your upper back, shoulder, or neck. The trigger points may feel especially painful when touched, and the pain may radiate beyond the immediate area.
There are two types of trigger points: active and latent. Active trigger points hurt when you move. Latent trigger points only hurt when pressure is applied along the raised part of the muscle.

Active trapezius trigger points may cause:
Pain - burning, aching and nagging in sensation. This pain may be localised to the area of the 'knot' as well as refer to other areas of the back, neck or arm.
Headaches- Trapezius trigger points often can be the cause of persistent headaches located over the temples, ear and base of one's head.
Jaw pain
Stiffness of the shoulder and/or neck
A feeling of 'heaviness' of the head
Fatigue
Anxiety
Poor sleep
Paresthesias

🌱Interferential current therapy (IFC) is the most common type of electrical muscle stimulation (EMS) used to treat chronic and acute pain.

🌱This non-invasive therapy has minimal side effects. By utilizing higher frequencies than other forms of EMS, IFC goes past the surface of the skin. It penetrates deep into the body to target the source of pain, without causing discomfort.

🌱Interferential current therapy works by sending small amounts of electrical stimulation to nerve endings and damaged tissues (muscles, ligaments, tendons, fascia) in the body. This type of stimulation releases endorphins to promote recovery while increasing blood circulation to the area to support the body’s natural healing process.
