I have noticed so many people suffering in silence due to venous/ arterial insufficiency to lower limbs (ibekelo/ umeqo). I have met people who started with treatment and decided to stop due to various reasons but mostly due to financial constraints. Some of them have given up going to social gatherings due to smell. Well, I will advise and help to treat these ( treatment will be possible to those closer to me in Roodepoort). Those who are far I can guide as to how to go about getting rid of these painful sores. I also supply elastic compression stockings to those in need at a minimal charge. We know , there's someone who has had a bandaged leg since we know them in our communities. This is for free!

Venous ulcer progress. First consult challenges were infection and intense pain. Couldn't walk or put the foot down and the pain was written all over the patient' s face. She also couldn't sleep and looked exhausted. Very anxious due to her last encounter was with 'scrubbing' of her ulcer by the health provider she saw. Reassured and counselled her about the long and winding journey of treating this ailment.No matter how sloughy a wound/ ulcer is, advanced woundcare techniques use modern painless desloughing presently. It makes the treatment easy to have the involvement of family members in this aspect. After two weeks the patient is painfree, walks around effortlessly and enjoys her sleep at night. Chronic venous ulcers are still poorly treated and are really a nuisance to the sufferers. Knowledge,passion,experience and patience are all needed in treatment thereof. Health providers should be able to detail the treatment needs as they are to the patient. Never promise a quick solution to the problem as these ulcers may take months with patient compliance or even years if patients do not comply. Main aspects to sort out first are:

1. Pain. This is very severe at night and it needs is relieved by offloading of venous stasis and pain killers. Pain deprives patients of quality sleep and rest that is vital for wound healing.

2. Infection. Treated with proper cleansing of wound bed/ debridement when necessary. Antibiotics may be of utmost importance here to tackle this.

3. Underlying diseases. All should be stabilized and patients referred for treatment if not on treatment. Encourage patient to adhere to treatment as required.

4. Odor/ Smell. The psychological impact of this results in patients isolating themselves from the rest of the world. Even at home family members may find it difficult to tolerate the smell. The smell is eliminated by some advanced wound care dressings/preparations.

5. Dressings. These are carefully selected according to how the ulcer/wound presents. No two wounds/ulcers are the same, so different dressings and bandaging are applied here.

Advanced Woundcare. What exactly does this entail? Not all of us understand what this means. Everyone of us has seen a wound/ in his life and most healed. So why the fuss?
All changed with diseases profiles,healing wounds became difficult. Lowered immunity, poor lifestyles, chronic diseases and ignorance are a some of the reasons that also contribute to wounds not healing.

Unfortunately, even with health workers, there is lack of interest in this aspect as it is associated with uncleanliness accompanied by malodour. Remembering during the days when all wounds were dressed with same material and ointment, they were healing miraculously but not now.

Research has been carried out and modernized and sophisticated dressing material has been manufacturef. How does one then choose amongst all presentations from various fancy and sometimes expensive dressing? In depth knowledge and experience will make one know which dressing to choose for a particular wound. Wounds/ ulcers never present the same, each and everyone of them is unique.

A wound becomes an ulcer if it takes more tha 14 days to heal. An ulcer then need expertise and investigations to ascertain the reasons of the delayed healing
