Let’s talk CERVICAL CANCER

- Cervical cancer…. which is the cancer occurring on the cervix or simply mouth of the womb is getting common

- In SA, it’s impact is causing serious family disruption and leaving young kids orphaned at serious young age

- What causes cervical cancer or risk factors?

First and foremost is due to a s*xual transmitted virus called HUMAN PAPILLOMA VIRUS ( HPV)- 99% of this cancer is due to s*x

And everyone can get it during their s*xual lifetime….
But you are at risk if you 👇

- you are HIV positive with uncontrolled disease
- immunosuppressed eg uncontrolled diabetes, cancer on chemotherapy, and other immune suppressing medical conditions eg SLE etc
- Smoker

How to prevent CERVICAL CANCER?

1. PRIMARY PREVENTION

- Vaccine to 9-12 years kids: there are vaccines…. Cervirax, Gardasil, …. With USA advocating for Gardasil -9 …. Which covers for most strains of HPV and Ge***al warts

Primary prevention targets women who have not engaged in s*xual debut

Smoke cessation etc

2. SECONDARY PREVENTION

This target all women who are having s*x…. If u a having s*x or had s*x in your life…. U need to be screened by tests below

- Papsmear or HPV Testing

The most commonest HPV for cervical cancer is type 16 and 18

Please get papsmear regularly to avoid this preventable cancer ladies

3. TERTIARY PREVENTION

- Papsmear done: now u have cells putting u at risk of cancer and or very very stage cancer… and that’s can be treated
- don’t get here… it’s not funny … try and be at number 2

UNFORTUNATELY END STAGE

- very common in our black society
- we have more 20-45 women dying of this condition than ever before….in our South African black communities…. and most of Gynae wards full of this age group….and 95% HIV positive

- regulations: all HIV positive women or high risk eg Diabetes/ cancer / or all immunocompromised women should have PAPSMEAR EVERY YEAR… due to aggression and quick cancerous activity of HPV on this group

IN SA we are now seeing 25-30 years coming to our facilities due to this preventable disease

Let’s talk fertility

Fertilization and making a baby is the most difficult task for human beings than all mammals combined

Humans nowadays struggle to form a baby than all other mammals … that’s scientifically proven

The global human fertility rate is approximately 2.2 births per woman in 2024, a decline from around 5 in the 1960s, and is projected to fall below the replacement level of 2.1 by the mid-2050s

Here are reasons

FEMALE

- Delayed childbearing: females have limited number of eggs and delaying fertility contributes to infertility
… by age of 35 -40 the rate of achieving fertility is extremely going down and after 40 years…. extremely low … only few can naturally form a baby

- Healthy conditions: eg Obesity, PCOS ( polycystic ovarian syndrome & Endometriosis), diabetes and hypertension etc make fertility rates to extremely go down … due to inability to ovulate

- Infections: Pelvic inflammatory disease and blocked fallopian tubes …. contributing massively … STIs as the main cause

- female organ structure: eg anything interfering with female anatomy structure will cause infertility eg FIBROIDS, Polyps , Adenomyosis, …. Remember some females might not be having any ge***al structures at all eg no womb at all

MALE

- Mainly inability to produce s***ms or if produced ? Abnormal s***ms….. inability to produce s***ms could be genetic or previous head trauma etc…. Where influential brain hormones are completely destroyed non existent to relay message to testicles to produce testosterone hormone which is the main hormone to produce fertility capable s***ms

- Just like females, Infections can STIs can destroy and block tract from testicles to end of p***s and men could be forming s***ms but completely blocked to come out…. Remember… fluid which is coming out from the p***s is not s***m but semen…. There is a big difference between the two

- Others: undescended testicles, genetic, and varicocele

ALL MEN / FEMALE COMBINED FACTORS

- Environmental: Excessive alcohol, smoking, obesity, Drugs like chemotherapy & anabolic drugs, excessive heat exposure eg too tied underwear’s or jeans for males etc

Conclusion

- Humans nowadays struggle to conceive
And this trend will continue…. Unfortunately families always have high expectations for us and most of the time relationship separations is due to one of this factor………. Inability to form a baby
