Beauty, Metabolism & The Thyroid: Moving Beyond “Normal”

In my practice, I often see the signs of hypothyroidism written on the skin and face long before a patient feels “sick.”

Hypothyroidism slows down your metabolic rate, impacting everything from cellular turnover to how your body processes nutrients.

The Aesthetic Red Flags:

• Dry, Flaky Skin: A compromised barrier that struggles to retain moisture.

• Puffy Face & Eye Bags: Fluid retention (myxedema) that can mask your natural facial contours.

• Hair & Nail Changes: Thinning hair and brittle, slow-growing nails.

Why “Normal” Isn’t Always “Optimal”

If you have these symptoms but were told your labs are “normal,” it’s time to look at the optimal ranges. Conventional medicine looks for disease, but we should aim for peak wellness.

TSH: While the conventional range goes up to 4.5, most patients feel best between 0.5 – 2.0 mIU/L.

Free T4 & T3: Essential for metabolism—Free T4 should ideally be 1.0 – 1.5 ng/dL. (Or 13 - 19 pmol/L)

Reverse T3: High levels (above 18 ng/dL or 270pmol/L) can act as a “brake,” blocking your active hormones.

The Conversion Factor

Even with a healthy thyroid, your body must convert T4 (inactive) into T3 (active). This process can be hindered by insulin resistance, high cortisol, or deficiencies in Zinc, Selenium, Iodine, Iron, Magnesium, and Vitamins D & B12.

Always check your laboratory units (e.g., pmol/L vs ng/dL) and ask your doctor to test for Thyroid Antibodies(TPOAb & TgAb) for a complete picture.

Don’t settle for just being within range.

Finally heard the words ‘you are an Ironman ‘!
But did not expect to be 3rd in my age group on my 1st Ironman.
Was rough sea conditions, strong wind on bike and the run was a long suffer!

I wanted to see how much I can challenge my body. We have been given such an amazing body that can be trained and developed to achieve amazing extremes.
But I quickly realised it’s more than just physical. It’s a huge mental challenge, to@push yourself through 3,8km swim, then 180km bike and the toughest mental challenge is the 42.2km run!

Hope my sharing inspires you to challenge yourself, and to realise the wonder of our amazing bodies. Respect it, nurture it, train it!

Anything is possible!
Journey to the full Ironman this Sunday.

I have always loved sport and training, especially cycling, but for more than 15 years my work, business, family and teaching priorities made me think I don’t have time for training for races.
Lockdown in 2020 made all of us rethink everything.
I decided I am going to work on this passion for competing in races again. Started training more, cycling and running. My husband decided to try an Ironman 70.3 race in 2021 as a challenge, I could not join him because I did not know how to swim !!!
I trained with him but started doing Duathlon races when he did triathlons. I enjoyed it so much and decided to learn to swim in 2021 in the 2nd lockdown.
This was soooo challenging!
Did my 1st triathlon (but not sure how I did the swimming part) 🤷🏻‍♀️
My husband always pushes me and entered me for my 1st Ironman 70.3. (1.9km swim, 90km bike, 21km run).
I prayed so much for the swim to be miraculously shortened, as I knew I would not make the 1.9 km swim. They shortened the swim just before the start due to bad weather 🙏🏻 Sorry other athletes, it was my fault in 2023!
But enjoyed it and motivated me to train more. Did more triathlons and 6 more Ironman 70.3 races and joined Sterk Span triathlon coaching club.
Qualified last November for the world championship Ironman 70.3 in Nice September 2026!!! (This is for age group athlete qualification - I am definitely not a pro athlete)

And now taking on the full Ironman this Sunday for the 1st time!
Very scared but excited.
🏊🏻‍♀️3.8km ocean swim
🚴🏻‍♀️followed by 180km bike
🏃🏻‍♀️and then off the bike for a 42.2 km marathon run!

Crazy when you think about it.
And the biggest challenge that I have taken on in sport.
No excuses!
I made time for training in my very busy schedule (full time doctor, teaching locally and globally with lots of travelling, mother of 3 and a husband as a training partner)
I had neck problems, congenital nose medical problems that stopped me for years from trying to swim. So never swam as a child.

If I can do it, so can you !

Back to basics with the next generation of aesthetic doctors!
Today marked another incredible session teaching anatomy to our new cohort of doctors embarking on their 2-year Advanced Diploma in Aesthetic Medicine.


There is truly no substitute for the real thing. I had the privilege of demonstrating the intricate facial layers—from the muscles and fat pads to the delicate network of arteries and nerves—using a fresh anatomy specimen.

Why does this matter?
Precision isn’t just an art, it’s a science.
Deep anatomical knowledge is the foundation of patient safety and natural results.

For example: Take a look at the origin of the corrugator supercilii muscle on the orbital rim. (Marked in green)
Understanding exactly where this muscle sits below the superciliary arch is the secret to successful botulinum toxin treatments. If you know the anatomy, you can effectively treat frown lines while significantly reducing the risk of heavy or sagging brows.

A massive thank you to the UP Anatomy Department for providing this world-class educational environment. It’s an honor to help these doctors build their journey on such a solid foundation.

One week to go 😬 for my 1st full Ironman!
Lots of mixed feelings:
Doubt and confidence
Fear and courage
Excitement and ‘what on earth are you doing?’

The leaves in my back pocket is not my ride nutrition 🥬
I bought some Morogo next to the road for R10!! ( that’s 0.50 euro).
Morogo is also called African spinach. 🇿🇦
It’s a popular African cuisine dish in Southern Africa, similar to kale or spinach, but better!
It’s a superfood full of vitamins, iron, calcium and protein.
Really delicious, especially when made the typical way with onions and tomatoes.
Perfect to boost me for this last week.
