The Safety Pyramid – Understanding How Small Acts Prevent Big Accidents ✋
The Safety Pyramid (Heinrich Pyramid) shows the link between minor incidents and serious or fatal accidents — proving that controlling the small things prevents major losses.
🔹 Key Levels:
1️⃣ Unsafe Acts / Conditions – Unsafe behaviors or workplace hazards (e.g., no helmet, slippery floor, exposed wires).
2️⃣ Near Misses – Incidents that almost caused harm but didn’t. Always report them — they’re warning signs.
3️⃣ No Treatment Cases – Very minor discomfort or pain that needs no medical care.
4️⃣ First Aid – Minor injuries treated on site (small cuts, burns, etc.).
5️⃣ Medical Treatment Injury (MTI) – Injuries needing external medical care, but the worker returns to duty.
6️⃣ Restricted Work Injury (RWI) – Injuries limiting the worker’s ability to perform all tasks.
7️⃣ Lost Time Injury (LTI) – Injuries causing one or more days of absence.
8️⃣ Fatality – The most severe outcome, which every safety system aims to prevent.
