*Postural Abnormalities*

Posture refers to how a person holds and positions their body during sitting, standing, lying, walking, and lifting.

An abnormal posture is any deviation from normal alignment and can lead to significant musculoskeletal problems over time.

🙆Common postural abnormalities include:
*Kyphosis*
*Scoliosis*
*Hyperlordosis*
*Flat-back posture*
*Sway-back posture*
*Forward head posture*
*Rounded shoulders*
*Upper crossed syndrome*
*Lower crossed syndrome*
*Handedness-related postural imbalance*

*Severe Low Back Pain After Prolonged Standing: A Case Report*

Recently, we discussed the relationship between posture and back pain. One key takeaway was that posture alone does not directly cause back pain. Rather, back pain usually results from a combination of several factors. We were also advised not to remain in one position for prolonged periods, even when the back is properly supported. Hence, the best posture is one that is frequently changing.

This concept proved very true in my own experience.

Three weeks ago, I began my internal clinical attachment feeling healthy, fit, and energetic. I was posted to work with the Ponseti team, where we manage both ward cases and booked clubfoot cases.

Our primary activities include cutting and removing POP casts, recasting, and assessing new clubfoot cases. These tasks require prolonged periods of standing and sitting.

On one particular day, the patient load was quite heavy. We spent close to three hours continuously standing while cutting POP casts and recasting after removal. At the time, I felt no discomfort. I was healthy, active, and experienced no back pain whatsoever.

However, a few days later, symptoms began to appear. I developed severe low back pain that significantly affected my posture. Standing upright became difficult, and bending was extremely painful.
As a physiotherapist, I can confidently say this was not mild discomfort. It was severe low back pain, rated 9/10 on the Visual Analogue Scale (VAS).

What is important to note is that despite being healthy, physically fit, and maintaining what I believed was good posture during the activity, I still developed significant back pain.

This further reinforces why we emphasize movement, even when good posture is assumed. Regularly changing positions is crucial in preventing back pain.

Many people believe that poor posture is the main cause of back pain. However, current understanding shows that the relationship is more complex.

This short guide explains it in simple terms.

*🧘‍♀️What is posture?*

Posture refers to how you position your body during daily activities such as:

* 📌Sitting
* 📌Standing
* 📌Walking
* 📌Using your phone
* 📌Lifting objects

Posture is not fixed. The human body is designed to move and change positions.

*🙆A common misconception*

It is often said that sitting or standing “incorrectly” will damage the spine.

The truth is:

* There is no single posture that automatically causes back pain
* Some people with “poor” posture have no pain
* Some people with “good” posture still experience back pain

👉Posture alone is not the main cause.

How posture can be related to back pain

Posture becomes important mainly because of how long and how often positions are maintained.

☑️Staying in one position for too long

* Holding any posture for long periods can strain muscles and joints
* This includes both slouched and upright positions
* Muscles become tired when they are not allowed to move

Any posture held for too long can cause discomfort.

☑️Lack of movement

* The spine is designed to move regularly
* Movement improves blood flow and reduces stiffness
* Remaining still for long periods increases the risk of pain

Movement is more important than perfect posture.

☑️Stress and muscle tension

* Stress can cause muscles to tighten
* Tight muscles may change posture and increase discomfort
* Back pain is often influenced by both physical and mental factors
