BABY IS IN BREECH PRESENTATION - WHAT DOES IT MEAN?

At term your baby should be head down. The presenting part of the baby should be the head. In medical terms "occiput anterior" . If the head is delivered first the rest of the body easily follows

But unfortunately it is not the case for every one. Some baby's present even at term with either the buttocks, hand, foot, shoulder and in some cases in a transverse lie.

This makes normal delivery in some cases a bit difficult.

Thanks to our midwives and doctors. A lot of these cases are detected earlier through palpation of the abdomen. Scan is also requested and that also helps to diagnose most of these cases before the birth of your baby

If a breech birth is diagnosed earlier, within the second trimester. There is a likelihood for baby to turn into the right position before birth. Your midwife will teach some exercise that will be helpful to you.

If detected late or baby does not change position at term, It's important to discuss mode of delivery with your health care provider.
Natural delivery may be allowed depending on findings like baby's weight, mothers parity, skill of the healthcare provider among others.

In some cases caesarian section will be requested to save both the life of the baby and mother.

It's dangerous to allow traditional birth attendants to try to turn the baby. It has led to serious problems and even death of some mums. Do not allow this!

Cooperate with your health care provider to provide the best solution.

DOES HAVING A BIG ABDOMEN MEAN MY BABY IS GROWING WELL?

📍Many women are concerned about the size of their bumps during pregnancy. They are always asking if my bump is small and it is not growing as big as I expect. Will it affect my baby? Is it safe?

📍The answer is your fundal height is what usually gives a midwife a clue as to whether the baby is small for date or big for date. Fundal height is what you see a midwife using tape measure to measure the bump. So it does not really depend on the appearance of your bump. A scan can also be done to determine your fetal weight. Your midwife or doctor will alert you when there is a deviation.

📍At birth your expected fetal weight considered normal is 2.5 kg to 3.5 kg. Anything less or more is considered as underweight or overweight.

📍Babies that are born and are overweight or big come with their own problem. Some include respiratory problems , birth injuries, hypothermia, hypoglycemia/reduced glucose level in the blood, still birth etc

📍Before your due date your doctor will request a scan. This scan can predict your baby's weight. Together with measuring your fundal height, a doctor may diagnose that you are likely to have a big baby.

📍Birth has to be done by a skilled attendant in a hospital. If the baby is very big, the doctor may request a caesarian section.

📍Mothers with gestational diabetes are likely to have big babies and it is important that birth is done in a hospital with a neonatal intensive care unit to resuscitate the baby where needed. During pregnancy glucose should be controlled to prevent complications

📍Eating a well balanced diet, exercising and taking prescribed medication can help you get a healthy weight for your newborn

📍After reading this, I hope you see that having a healthy baby does not mean the baby should be big or your bump should be big!

HOW BREASTFEEDING CAN HELP STOP BLEEDING

Did you know breastfeeding right after your baby is born can help the uterus to contract and thereby help stop bleeding? Yes, as your baby sucks oxytocin is released and this hormone helps the uterus to contract.

It is one of the reasons why early initiation of breastfeeding is important to both mum and the baby.

It also explains why as the baby sucks the mum experiences what we call after pains. Pains in the lower abdomen as the baby is breastfeeding. It is as a result of the baby sucking and the oxytocin released to help the uterus to contract after birth.

Afterpain can be controlled by taking analgesics, exercise and also rest. Excessive pain is not normal and should be reported.

Bleeding excessively after birth should also not be considered as normal. Do report early to prevent further complications

BIRTH ASPHYXIA - WHAT YOU SHOULD KNOW

This occurs when the amount of oxygen, and nutrients reaching the baby's brain is reduced

Certain conditions can cause the amount of blood supply getting to the baby to be reduced causing fetal distress and later result in asphyxia. (Low oxygen supply to the brain) it can occur just before birth, during labor and when baby is born

Examples of conditions include, prolonged labor, placenta abruptio, reduced oxygen in the mum, infections etc

Some signs baby will show include
✔️Not breathing or very weak breathing.
✔️Skin color that is bluish, gray, or lighter than normal.
✔️Low heart rate.
✔️Poor muscle tone.
✔️Weak reflexes.
✔️Meconium stained liquor
✔️Seizures.

If it's less severe, the baby will be resuscitated and helped to breathe until breathing is well established. In severe cases more interventions will be needed.

Report early if you suspect the baby is in distress. In utero there will be reduced movement or excessive movement.

It's Important to have skilled delivery, deliver in a hospital that has items ready for neonatal resuscitation and if the baby is still in utero an emergency cesarean section will be done to save the baby. Your cooperation at this time is extremely important. It can lead to complications like brain damage. Do seek help quickly

VERNIX CASEOSA- WHAT YOU SHOULD KNOW

You see that whitish substance on your baby's after it's born? What did you make out of it?
Some even think it's dirt or as a result of eating ayilo/shile/Pica. But this is not the case.

This is what you should know.
When your baby is born, he might be covered with a thick, white, cheese-like layer called vernix caseosa. The older the baby is the less you find it on their skin. It begins to form around 25 weeks and desolves into the amniotic fluid with time.

It's benefits

In the womb, vernix helps protect your newborn’s delicate skin from the acidic quality of the amniotic fluid

It hydrates her skin, insulates her body, and maintains a proper—and comfortable—temperature in utero.

Vernix also does doubles as a lubricant, helping your baby slide out of the birth canal a bit easier.

It gives that new born smell your baby has, it helps mum and baby to bond

The World Health Organization advises delaying baby's first bath for at least 6 hours, although 24 hours is ideal. This will help baby to adjust to the new environment and the vernix plays a role as well

Wash VERNIX off right away if Baby has meconium staining, or if the mother has HIV or hepatitis.

Do not force to remove all at a time. With time all will be removed.
