History of Blood Transfusions and Grouping

The first blood transfusion took place in 1667 where blood from a sheep was transfused into a 15 year old boy. However after an influx of patients dying from procedures like this it resulted in being banned.
The ABO Blood Group System is a vital blood group system for transfusions.
The system was discovered in 1900 by Austrian Scientist Karl Landsteiner who identified the first three types; A, B and O. In 1902 two Scientists Afred Von Decastello and Adriano Sturli discovered the fourth and last type; AB.
In 1940 the Rh factor was discovered after observing consistent bad reactions when blood was transfused. His is the ‘+’ and ‘-’ part of the blood group. So if you are A+, you have the Rh D antigen but if you are A- you don’t have the antigen.
There are actually more than eight blood types but that does not affect blood transfusions from human to human currently.

The Dangers with Blood Types

If given the wrong blood in a transfusion you can get an ABO incompatibility reaction, which is although rare nowadays, it can be serious and fatal.
During an ABO incompatibility reaction the red blood cells break down and cause blood clotting throughout the body. This can shut off blood supply to vital organs leading to a stroke then more serious consequences such as organ failure and bleeding out.
Although a very serious reaction, if treated properly and quickly, a full recovery should be the outcome.

In pregnancy it is very important to know both your ABO blood type and your rhesus status for complications can happen if your fetus has different blood stats to you.
For example if you are a prospective mother with blood type A- and the fetus is A+; your immune system can react to the antigen in the fetus’ blood causing the release of antibodies to fight against them, thus treating the fetus as a foreign invader to the body. This is known as a Sensitising event.
The outcomes of pregnancy where a sensitising event has taken place are:
• The baby suffers from anemia, which depending on the severity can lead to heart failure.
• The baby becomes jaundiced. Causing the liver to fail. (this is known as haemolytic disease of the newborn (HDN)).
• HDN can cause permanent brain damage and neurological problems like cerebral palsy.
However with effective treatment and diagnosis, HDN is now very rare and can be treated easily with routine injections and close monitoring.

While there is more to explore when it comes to blood types, knowing just your blood group and Rhesus factor can make a lot of processes easier. Blood grouping can be done in many ways, but the most common way is with use of a Blood Grouping Reagent kit, which sorts out blood by type.

What is a Coombs' Test?

Patients with insufficient amount of red blood cells in their body may experience fatigue, tiredness, cold hands and feet, pale skin and shortness of breath. Low red blood cells lead to the condition of anaemia, which may require a blood transfusion.

To find out what kind of anaemia you have, a Coombs’ test is carried out.

What Does a Coombs’ Test Do?

The test will determine if the body contains certain antibodies in the blood stream. When your immune system detects that something is or could be harmful to your body, it produces the protein antibodies.

However, the immune system could be wrong and create antibodies by incorrect detection, which can cause other health problems, and not destroy what is harmful.

The Coombs’ test will determine if your antibodies are attacking and destroying your own red blood cells.

Types of Coombs’ Tests

There are two types of Coombs’ tests, the direct and indirect tests.

• The direct test is the most common; it is done from taking a blood sample from the patient via an injection. This test will check if any antibodies are attached to the surface of your red blood cells.

The red blood cells are washed (removed of patient serum), and then separated by antihuman globulin. If there is immunoglobulin in the red blood cell surface in-vivo, the direct Coombs test will show positive, as the antihuman globulin will have agglutinated.

When red blood cells are being destroyed, a condition called haemolytic anaemia is created; it is lead by haemolysis and is when the bone marrow activity cannot compensate for the increase in loss of red blood cells.

• The indirect test is used to detect in-vitro antibody-antigen reactions, and to check for unattached antibodies that are floating in the bloodstream as apposed to being attached to the red blood cells.

Prior to blood transfusions, an indirect Coombs’ test will be carried out to see if there are low concentrations of antibodies in the patient’s serum.
Seremed supplies Coombs made by Lorne Laboratory, Coombs’ control cells are made up of a 2-3% suspension of single donor group O red cells that have been removed of all blood group antibodies and resuspended in a preservative solution.

They are used after the Coombs’ tests to check the test was done properly when the test results are negative. They are cells coated with IgG antibody, and if agglutination continues after Coombs’ control cells check, this verifies the negative results

Febrile Antigen Kits (WIDAL)

Lorne Stained Febrile Antigens are for the detection of certain Salmonellae, Richettsiae and Bruchellae pathogens.

How Lorne's Febrile Antigen Kits work

The antigens are suspensions of killed bacteria, stained to enhance the reading of agglutination tests. The blue stained antigens are specific to the somatic “O” antigens and the red stained antigens are specific to the flagellar “H” antigens.

Suspensions of Proteus OX2, OX19 and OXK are used to detect rickettsial antibodies.
Rose Bengal is a slide agglutination test for the qualitative and semi-quantitative detection of anti-Brucella antibodies in human and animal serum. The stained baterical suspension agglutinates when mixed with samples containing specific IgG oro IgM antibodies present in the patient sample.
