The movement system is a physiological body system in which the Physical Therapist is the specialist. It’s viewed as any other bodily system (like the cardiovascular system, respiratory system, etc…) and treatment involves movement!

A basic tenet of the movement system is that our repeated movements and sustained postures influence our musculoskeletal system and shape our relative stiffness and strength.

What is an example of a movement impairment?

One commonly seen example of movement impairment is the second photo. All of us sit down and stand up several times daily, which is in essence, a functional squat. Generally, the photo shown on the left is a more optimal alignment/movement pattern. This person likely has more balanced strength & flexibility than the person on the right.

If the person on the right is also having pain moving between sit to stand (and other similar functional movements), then this movement should be directly addressed and become part of a PT treatment.

In keeping with the example above, we may also see this same individual habitually sit in a position similar to the picture shown below. This would be an example of the influence of static postures on the movement system. As this photo is somewhat of an exaggerated position, it’s easy to see how sitting this way for long stretches of time would contribute to further muscle imbalances.

PT treatment for this individual would include education on improved seated positioning, plus retraining of the functional squat or sit-to-stand pattern to minimize stress on the painful structures. This helps to reduce pain short term and hopefully form a more optimal movement pattern to prevent return of symptoms in the future. Treatment would also address deficits in mobility (joint or soft tissue) and neuromuscular control that are contributing factors to pain.

The next photo shows suboptimal alignment; however a comprehensive evaluation helps to determine what an individual’s optimal movement pattern should be, tailored to them. For some individuals, the first picture shown of the squat motion (on the left) may not be ideal due to structural issues like tibial torsion or femoral version.

The best way to know what could be contributing to your pain is to seek care with a Physical Therapist that will take time to evaluate you and look at your daily movements and positions. Although the onset of pain might be sudden, unless there is a traumatic event that caused the injury/pain, it’s likely the result of stress over time relating to how we move.

At our last clinical staff meeting, Kate presented a recent study on the classical formula, Xiao Yao San and its ability to treat depression. Swipe to read key insights and read below for more detailed info!

- Participants were recruited from November 1, 2020 to December 31, 2021 from the outpatient clinics of Peking University Sixth Hospital and the First Affiliated Hospital of Jinan University.

- 10 healthy volunteers and 41 patients were recruited
- The 17-item Hamilton Depression Rating Scale was used as the primary outcome measure, a TCM syndrome score scale designed for the study was the secondary outcome measure used

- Participants were assessed at 2, 4, 6, and 8 weeks using both scales
The study was double blind, randomized, and placebo-controlled

Results:
- XYP (Xiao Yao Pills) alleviated mild and moderate depressive disorder
- XYP relieved psychomotor retardation symptoms in patients with mild/moderate depressive disorder
- The therapeutic effect of XYP on psychomotor symptoms decreased with age.
- XYP partially restored the expression of depression-related genes
- XYP affects DNA methylation associated with depression-related genes
- XYP may regulate DNMT1 to rescue abnormal gene expression

Conclusion:
Xiao Yao San is a useful tool to combat depression in adults under 45 years of age when prescribed by a licensed acupuncturist/Chinese herbal medicine practitioner

Spring marks a time of metamorphosis and enlightenment. During this time, we make several changes to our lifestyle to help us transition from the heavy winter to the light, airy spring. Normally, people think about ‘spring cleaning’ to prepare for the change in season, but one of the areas in which we should think about change is our diet.

Winter weather causes us to gravitate toward heavy, building foods (meats and cheeses) because these fatty, protein-based foods help us regulate our body temperature. Spring weather, on the other hand, does not take the same toll on our bodies, so it requires us to move away from such heavier foods and to develop a mind-body connection through food cleansing.

Food consumption should be reduced to the simplest foods that are in season. Bitter greens, such as kale, swiss chard, and dandelion, can help cleanse the body for spring. Fruits and vegetables, nuts and seeds, and whole grains provide a milder cleanse for those who are not accustomed to a such light fare.

If you live in the South, you can begin cleansing early on in the season. Those who live in the North, might want to wait until the warmer weather of May. Please, however, consult a professional before beginning a cleanse and be careful not to cleanse too aggressively if you are not accustomed to it.

Seana grew up in New Hampshire but decided to make Maine her permanent home in 2021 after many years of exploring different parts of the US. A first generation American to a German family, her values are heavily influenced by her native culture, with a specific emphasis on holistic health and an interest in natural remedies. Seana believes strongly that massage therapy is an indispensable part of healthcare and should be seen as a necessity rather than a luxury.
