PIERCING KELOID SCAR RESULTS

These images show our 28 year old patient before and 4 months after 3 laser and steroid treatments to the thick spherical keloid scar on the back of her ear, created by a helical piercing.

This scar is much smaller, softer, less red and has flattened by over a centimetre.

Many ear keloids shrink enough with laser and steroid alone that patients can avoid surgery for the keloid altogether.

KELOID ARM SCARS

These images show our 25-year old patient before and 3 months after three laser and steroid treatments to the keloid scar on her right upper arm.

The scar is much flatter, less red and less itchy than previously. She is now using silicone taping to maintain the gains achieved with treatment.

Why can’t you just cut out a keloid scar?

We have been asked this again many times at The Scar Clinic Perth, so thought it was worth taking the time to re-explain:

If you cut out a keloid with no planning for post-operative steroid injection, laser, pressure or radiotherapy then the chances of that keloid returning the same size or even bigger are almost 100%

Remember, keloids are often caused by minor trauma, yet grow very large.
Imagine what a fresh surgery scar would do!!

This doesn’t mean that we don’t ever use surgery to treat keloid scars, but it is more successful in certain types of keloids (e.g. earlobe keloids) where you can apply a pressure button.

CO2 laser combined with steroid has been a game-changer for most keloid improvement treatments and has very little risk associated.

Yes, it’s true. Unborn babies before 24 weeks gestation heal without leaving any visible scar.

Nobody knows the exact mechanisms but differences in the anatomy of the skin and its response to wounding, regulation by cell chemicals, gene expressions and different collagen types produced are proposed reasons.

Whilst we can’t turn back the clock on our skin to mimic an unborn baby’s, scientists such as those supported by and continue to try to recreate a wound healing environment which would reduce scar formation to benefit all types of trauma and wounds.

LEG KELOID SCARRING

These images show our 83 year old patient before and 4 months after a single laser and steroid treatment to the itchy, painful 15mm thick keloid scar on the back of her knee, created by a surgical wound 40 years before.

Previous steroid injections alone had not changed the scar.

After a single combined laser and steroid treatment she had resolution of the itch and pain.

She has achieved good flattening of this scar and the colour will continue to improve now that the thickness is improving.

She has decided to undergo a further treatment to try to achieve complete flattening.

What’s better? Laser or IPL?

Laser is beams of light which are:
- all one wavelength/colour (monochromatic)
- all the same direction (coherent) and spacing (collimated)

This makes it very powerful and it can vapourise, heat and explode things that it targets
- If the target is water then laser can powerfully treat collagen - thick scars, wrinkles, acne scarring etc.
- Laser can pe*****te much deeper than IPL and treat 4mm into the skin or scar.
- The most powerful class of lasers (class 3B and 4) which are ablative (remove layers of the skin/scar) require a laser-licensed doctor to perform the treatment.

IPL (intensed pulsed light) is not a laser - it’s a broad bandwidth of light that has a filter placed over it to then produce a narrower band of light waves to be absorbed by a range of coloured targets on the skin surface - e.g. red rosacea skin changes and fine blood vessels, brown superficial skin pigmentation changes or age spots
- It is not as powerful and does not require a license to operate.

They are both good treatments for different things and knowing the difference will help you choose what is right for you.

Scar Nutrition

At The Scar Clinic we are frequently asked “Are there changes I can make in terms of my diet to get better healing or scarring?”

Most people eating a well-balanced diet have enough vitamins and minerals to achieve good wound healing in a timely manner.

Some people (especially those with gastro-intestinal issues or who have had weight-loss surgery) choose to take vitamin supplements for reasons of insufficient intake or problems with absorption.

Some important vitamins and minerals in wound healing are:

Vitamin A - helps cross-link collagen
Vitamin C - required for making collagen
Vitamin E - acts as a membrane stabiliser

Zinc, Copper and Selenium - all important co-factors for enzymes involved in healing

Protein - low levels of albumin (made from protein in the diet) are associated with poorer wound healing

PLEASE NOTE: It is important to speak to your GP or health care provider before supplementing your diet with vitamins and minerals - some e.g. Vitamin A can be toxic if taken in high levels and if taken in pregnancy can affect development of unborn babies.

ACNE SCARS

These images show our 30-year-old patient before and 5 months after 4 laser resurfacing treatments to her acne scars.

Each treatment has built upon the last; sequentially making the scars less obvious whilst minimising downtime and risks of pigmentation change.

Whilst the scars have not disappeared, they are smoother and less visible due to the reduced contour abnormalities on the skin surface after laser treatments.

She had 5 days of downtime with each treatment, being completely healed and able to wear make-up at day 6 post-procedure.

CHEST KELOID SCAR

These images show our 40-year-old patient before and 9 months after a single laser and steroid treatment to the very large, raised, itchy keloid scar on her chest which has been present for 20 years and had previous steroid injections along with limited response.

After a single combination treatment the scar is now much flatter, paler and less itchy.
She has undergone another treatment to achieve further improvement.

Q: How many CO2 laser treatments will my scar need?

A: Depends on the scar (but most need between 1-3)

All scars are different and the speed and response to CO2 laser is different for each scar.

A smaller post-surgical scar which is only 2mm raised and itchy might only need 1 treatment to get flat, pale and asymptomatic.

A larger keloid scar which is over 10mm thick, itchy and sore will almost certainly need more treatments to get a good level of flattening and maintain symptom control.

Each additional CO2 laser treatment is based on the response to the last.

Our experienced scar surgeons can advise you as to the best timing and frequency of any treatment for your scars.
