AT-HM helps reduce risks, improve accessibility, and make daily life easier, so you can continue living comfortably in the home you love🏡

Under the Support at Home program, there will be dedicated funding for Assistive Technology & Home Modifications (AT-HM). That means essential changes like grab rails, shower supports or other safety upgrades won’t need to come out of your regular care budget.

Here’s how it works:
âś… Complete an assessment
âś… Receive a funding tier (low, medium or high)
âś… Use your approved funding within 12 months for equipment or home modifications

In this , we will be talking about the Single Assessment System (SAS).

The SAS is Australia’s new way of assessing your aged care needs. It helps you and your family get the right support at home or in residential care without having to repeat your information for every service.

With SAS, one assessment is shared across all government-funded aged care services, including:
🤍 Support at Home
🤍 Respite care
🤍 Residential aged care

Why it’s helpful:
✔️ You only need to tell your story once
✔️ Your care can change as your needs change
✔️ Assessments are consistent and clear, making it easier to get the support you need

1800MEDICARE (1800 633 422) is a free, 24/7 helpline where you can speak to a registered nurse about any health concern. The nurse will ask about your symptoms and guide you to the right care, whether that’s looking after yourself at home or getting help from a health service in your area. You can also access key health information, electronic prescriptions and more through the 1800MEDICARE app.
