It’s official! Starting tomorrow, November 18th, Life Labs will be offering lab services at the Health Hub every Tuesday from 8:30am - 12:30pm. This service is open to the public! Walk-in only and please be sure to bring your requisition

What is a Family Health Team

What is a Family Health Team?

Funded by the MOHLTC, a Family Health Team is an approach to primary health care that brings together different health care providers to co-ordinate the highest possible quality of care for you - the patient.

Designed to improve and expand access to primary care, most Family Health Teams consist of doctors, nurses, nurse practitioners, social workers, dietitians and other health care professionals who work collaboratively, each utilizing their experience and skills so that you receive the very best care, when you need it, as close to home as possible.

The CKFHT is not a walk-in clinic and only patients who are rostered to the physicians of the CKFHT have access to the programs and services of the CKFHT. This is the same for all Family Health Teams across the province.
