✨ What’s ACP?
Advance Care Planning is the process of thinking ahead about the health and personal care decisions you’d want made if you’re ever unable to speak for yourself. It’s about autonomy, dignity, and ensuring your wishes are honoured.

✨ Why does it matter?
For q***r and trans people, choosing the right person to represent you is essential. ACP helps you:
✔️ Document your wishes
✔️ Identify who can make decisions for you
✔️ Talk with your doctor, loved ones, and chosen family
✔️ Ensure your future care reflects who you are

✨ What is Green Sleeves?
Think of them as your healthcare passport; a place to store your medical info and ACP documents so your wishes are clear in an emergency or during a health decline.
