Advance Care Planning Canada

Advance Care Planning Canada ? Who would you trust to make your healthcare decisions? April 16th is the Canadian National Advance Care Planning Day.

It is a day for you and others to reflect on decisions you would like made about your health care, should you be unable to speak for yourself. Advance Care Planning Day provides an opportunity to open up a conversation with a loved one about their wishes for end of life care - or it might remind you to review your own plan.

"I realized I would now need to access not just some of their tangible documents but all of it."

Caregiving is a leap into the unknown, but it doesn't have to be a hunt for information. At ACP Canada, we talk about Advance Care Planning as part of a broader life care plan. Whether it’s financial, legal, or health-related, your wishes need to be accessible to the people who will speak for you.

If not you, who will have these answers? If not now, when will you start the conversation?

On March 6th from 10:00 AM – 11:30 AM, Coburg Public Library are hosting a free, interactive community session led by certified trainers from Hospice Palliative Care Ontario (HPCO). It promises to be an empowering workshop designed to put you back in the driver’s seat of your own healthcare journey.

Learn about your rights in the Ontario healthcare system, how to choose the right Substitute Decision Maker, and how to have those "courageous conversations" with your loved ones so your voice is always heard.

Seniors aged 55+, family members, and caregivers welcome. This is a drop-in session, but the information is life-changing. Come with your questions and leave with the confidence that your future care aligns with your values.
