Carotid paraganglioma. This tumour is similar to a pheochromocytoma of the adrenal gland. It is composed of chief and sustentacular cells arranged in a nested or "zellballen' architecture with a prominent vascular network and areas of hyalinized stroma. IHC would stain chief cells positive for chromogranin/synaptophysin and sustentacular cells positive for S100.

Gastric adenocarcinoma with signet ring morphology. Sections show benign glands with a diffuse stromal infiltrate of single cells. The neoplastic cells have abundant intracytoplasmic mucin and an eccentrically placed nucleus.

Barretts esophagus. This biopsy of the distal esophagus shows intestinal metaplasia of the normal stratified squamous mucosa into columnar epithelium with goblet cells. The most common risk factor is gastroesophageal reflux disease. Endoscopically it appears as a salmon colored plaque extending beyond the gastroesophageal junction. Progression to esophageal adenocarcinoma is a possible complication.

Epithelioid sarcoma. This tumour is a malignant mesenchymal neoplasm with epithelial differentiation. It is composed of small-medium epithelioid cells with eosinophilic cytoplasm and mildly atypical nuclei. Immunohistochemistry is positive for CD34, CK8/18, and negative for INI1 (associated with a SMARCB1 deletion).

Psammomatoid juvenile ossifying fibroma. This tumour is a rapidly growing benign fibro-osseous lesion that affects the extragnathic craniofacial bones of young patients. It consists of psammomatoid bodies embedded in a cellular fibrous stroma and within bony trabeculae.

Mucoepidermoid carcinoma of the parotid. This is the most common malignant salivary gland neoplasm. It has a solid to cystic architecture with variable proportions of squamoid-epidermoid cells, intermediate cells, clear cells, and mucin secreting cells. A t(11;19) translocation is seen in 55-65% of cases resulting in a CRTC1-MAML2 fusion favoring a better prognosis.

Adenoid cystic carcinoma of the breast. This is a relatively rare form of invasive breast cancer composed of luminal and myoepithelial cells. The histology can show cribriform, solid, or tubular architecture, and the morphology is similar to its counterpart in the salivary glands. The most common genetic finding is a chromosomal translocation resulting in an MYB-NFIB fusion.

Intravascular leiomyosarcoma. This spindle cell neoplasm is arising in the wall of the inferior vena cava. It is derived from smooth muscle cells and has a fasicular architecture. Subsequent images show highly pleomorphic cells and an atypical mitotic figure.

Synovial Sarcoma. This tumour is a malignant neoplasm of soft tissue. It can have many morphologies but shown above is a biphasic population of spindle and gland-like epithelial cells. Molecular testing will commonly detect a t(X;18) SS18-SSX gene fusion. Fun fact - despite its name Synovial Sarcoma does not arise from synovial cells, and the cell of origin remains unknown.

Malignant melanoma. In this section of skin diffuse sheets of atypical melanocytes are invading into the dermis with associated multinucleated giant cells. Immunohistochemistry is positive for Melan-A and SOX10.

Whipple’s disease of the duodenum. The small bowel is infected with the gram-positive bacterium Tropheryma whipplei and is characterized by foamy macrophages in the lamina propria that stain positive for PAS (diastase resistant).
