Our nurses, known as patient navigators and health care advocates, offer a range of services. They assist in communication among physicians and patients, helping clients to define health care goals, set agendas for doctor’s visits and shape expectations. We offer a free 30 minute, no obligation, phone consultation to see if our services are right for you. We'll follow up with a personal meeting to get you started. Afterwards you'll meet with your primary nurse to create your individualized health narrative.
