Welcome to Clifton Hospital

Clifton is a community hospital based in Lytham St Annes and is part of Blackpool Teaching Hospitals NHS Foundation Trust.

Overview

Clifton Hospital in-patient Wards provide a rehabilitation service for patients that are transferred from an acute hospital such as Blackpool Victoria Hospital or from their own home by one of the community health teams supporting them.

Each of the wards provide a nurse/therapy led holistic pathway for inpatient rehabilitation to people who are transferred to it for a period of rehabilitation or as part of a complex discharge planning process. Patients receive input from the multi-professional team to return home independently or with support from social services or care agency. Some patients may transfer from Clifton Hospital to care homes for ongoing care.

The approach utilised at Clifton Hospital is person centred, comprehensive and cohesive to enable the people of Blackpool and the Fylde coast to remain as independent as possible within the community.

Our aim

To provide an extended period of rehabilitation (usually up to a maximum of 28 days) in an inpatient setting for people who will benefit from assessment and therapy from specialist multi-disciplinary team.

The rehabilitation team for the inpatient beds at Clifton Hospital includes:

Clinical Matron

Advanced Practitioners

Ward Managers

Therapy Team Leader

Registered nurses

Physiotherapists and Occupational therapists

Nursing Associates and Assistant Practitioners

The team are supported by a group of dedicated health care assistants and therapy assistants.

Patients may also be supported by Speech and language therapists, Dieticians and Pharmacists if needed.

Clifton Hospital is proud to support the training and development of trainee advanced practitioners, student nurses, student physiotherapists, student occupational therapists, trainee nursing associates and trainee assistant practitioners. We are keen to help students and trainees develop their skills in a supportive and inclusive environment. All students and trainee are supervised by our registered staff.
