💊 Pharmacy First: the right care, closer to home

Pharmacy First allows people to get advice and treatment for some common conditions directly from a local pharmacy, without needing to see a GP first. Pharmacists are highly trained healthcare professionals who can assess symptoms and provide advice or treatment where appropriate.

You can use Pharmacy First for:

Sinusitis: aged 12 years and over
Sore throat: aged 5 years and over
Earache (acute otitis media): aged 1 to 17 years
Infected insect bites: aged 1 year and over
Impetigo: aged 1 year and over
Shingles: aged 18 years and over
Uncomplicated urinary tract infections (UTIs): women aged 16 to 64 years

A pharmacy is a good first choice when symptoms are mild to moderate, same-day advice is needed, or it is outside normal surgery hours.

Jess’s Rule supports patients, families, and carers to speak up if they are worried about a patient’s condition.

If concerns aren’t being addressed and something doesn’t feel right, Jess’s Rule provides a clear way to escalate concerns so the patient can be reviewed and supported.

The aims are simple:
👂 Making sure patients are listened to
⏰ Encouraging early action
🛡️ Improving patient safety and care

We encourage patients and families to always feel confident asking questions and raising concerns about their care.

In 2025, Castle Partnership completed a service delivery change with regards to the care we provide to our care homes.

We provide health care to the residents of Cromwell House, Corton House Care Home, Baytree House, Hillgrove and The Hawthorns.

We also provide healthcare to the residents of Cavell Court Care Home in collaboration with our colleagues at St Stephen's Gate Medical Practice.

Each of our care homes has a designated Advanced Nurse Practitioner, Clare Sullivan or Melanie Gorman who complete ward rounds weekly for the residents.

We also hold two weekly multi disciplinary meetings where our nurses meet with the care home, our GP Care Home Leads (Dr Kirsty Hewitt, Dr Lydia Serjeant and Dr Katie Voight), members from our Pharmacy team, our Primary Care Network Dietician and representatives from NSFT and other organisations, to consider the best care for each patient as an individual. These meetings are organised and facilitated by our Care Coordinator Team.

Our Care Home Team work closely with our Home Visiting Team to ensure any residents who become acutely unwell are visited outside of these ward rounds but continuity is maintained.

This change has been beneficial to our patients, their families, our staff and the care home staff and has seen a reduction in unplanned admissions.

Corton House Registered Manager, Jason Parker says “since the introduction of the fortnightly multi disciplinary meetings alongside our weekly ward rounds, we have found these to be extremely productive. The meetings have created several excellent outcomes including building on working in collaboration for the benefit of our residents, given opportunities to having a focussed and holistical approach to meeting individual residents needs including discussion from all prospective in relation to the persons health and wellbeing, medicine review, referrals to other specialised services alongside providing regular updates on residents who have been admitted to hospital. We are extremely thankful to be able to have this quality focused time.”

Pedro Campos, Manager at Cavell Court Care Home reccomended Clare Sullivan for an award for her dedication to her role - which she was presented with in December at the Castle Awards Ceremony.

The way in which we provide health care to our care homes has recently been recognised at a regional level and we are very proud of this achievement - a huge thank you to our dedicated Care Home Team - Nurses, Care Coordinators and GPs 🙏🏼
